摘要
目的探讨铅污染区儿童血铅与锌原卟啉、红细胞参数相关性。方法对云南某县铅锌矿区和县城非铅锌矿区2~9岁儿童共464人进行血铅(BPb)水平、红细胞参数及锌原卟啉(ZPP)水平调查,按血铅水平分为高、低血铅组并进行血Pb与红细胞参数(RBC、HB、HCT、MCV、MCH、MCHC、RDW)及ZPP相关性分析。结果儿童血铅浓度均值154.4ugL,铅中毒率79.1%,ZPP均值2.7uggHb。血铅>250ugL时,血铅与红细胞参数具有相关性(RR:0.637)。高铅组血铅水平与RBC、ZPP有较强相关性(r分别为:-0.975、0.957)。RBC随血铅水平增高呈高度负相关,血铅水平<200ugL时,与ZPP无相关性。血铅水平达~250ugL、~300ugL、~350ugL时,血铅与ZPP具有较强相关性(r分别为:0.567、0.93、0.886)并呈高度正相关。结论随血铅水平的增高,RBC与血铅呈高度负相关,当血铅水平>250ugL时,血铅与ZPP具有高度正相关性,而当血铅水平<200ugL时,与ZPP相关关系不密切,因此ZPP不能作为铅中毒筛查指标。
Objective To investigate the correlation between children' s Blood lead(Bpb) ,Zinc Proto- porphyrin(ZPP)and Red Blood Cell (RBC)parameter in a lead - polluted area. Methods Bpb, RBC parameter and ZPP level investigation were conducted on total 464 children(age 2 - 9) in a Pb - Zn mineral district and no Pb - Zn mineral district in its city of some county in Yurman province. Based on their BPb levels, they were divided into a high and a low BPb group and were conducted with the correlation analysis between BPb, RBC parameters(RBC, HB, HCT, MCV, MCH, MCHC, RDW)and ZPP. Results Children's Bpb level average is 154.4ug/L, the lead - poisoning rate is 79.1% and ZPP average is 27ug/gHb. When the BPb is great than 250ug/L(BPb 〉 250ug/L), BPb is correlated to RBC pararneter(RR:0.637). The BPb level inthe high BPb group is closely correlated with RBC and ZPP respectively(r: -0.975 and 0.957 respectively). RBC appears a highly negative correlation with BPb level increasing. It is not correlated to ZPP when the BPb level is less than 200ug/L. When the BPb level is reaching to 250ug/L, - 300ug/L, - 350ug/L, BPb shows a very. strong correlation with ZPP(r is 0.567, 0.93 and 0.886 respectively) and also appears a highly positive correlation. Conclusion With BPb level increasing, RBC appears a highly negative correlation with BPb.BPb becomes highly positive correlated with ZPP when Bpb 〉 250ug/L. However, when BPb 〈 250ug/L, the correlativity between BPb and ZPP is not close. Therefore, ZPP will be not able to be used as a lead - poisoning widely checkout norm.
