摘要 目的观察不同糖代谢状态下血清肿瘤坏死因子(TNFα)和游离脂肪酸(FFA)的变化,以探讨其在糖尿病发生发展中的作用。方法测定27例正常糖耐量(NGT组)、28例糖耐量受损(IGT组)和21例初诊2型糖尿病(T2DM组)个体空腹血清TNFα和FFA水平,分析其对胰岛素抵抗(Homa-IR)和胰岛β细胞功能(HBCI)的影响。结果①IGT组和T2DM组空腹TNFα和FFA明显高于NGT组(P<0.05);②NGT、IGT、T2DM组TNFα和FFA之间差异有显著性(r=0.701,P<0.01);③空腹TNFα和FFA与Homa-IR显著正相关(r分别为0.646,0.601,P<0.01),与HBCI负相关性(分别为r=-0·245,-0.244,P<0.05)。结论IGT和T2DM患者存在血清TNFα和FFA明显增高,且均与Homa-IR正相关,与HBCI负相关,提示高水平血清TNFα和FFA可能参与构成T2DM的病理生理基础,包括IR和β细胞功能障碍。 Aim To observe the changes of serum tumor necrosis factor alpha (TNFα) and free fatty acids ( FFA ) in different glycometabolic states and to explore their role in the development and progression of diabetes. Methods The serum levels of fasting TNFα and FFA were measured in 27 individuals with normal glucose tolerance ( NGT), 28 individuals with impaired glucose tolerance (IGT) and 21 individuals with diabetes (T2DM), and their effects on insulin resistance ( Homa-IR ) and isletβeell function ( HBCI ) were analyzed respectively. Rerults Serum levels of both TNFα and FFA were increased in T2DM and IGT compared with NGT subjects( P 〈0. 05 ) ; Correlation analysises showed that there were statistically significant correlations between fasting TNFα and FFA among NGT, IGT and T2DM groups ( r=0.701, P 〈0.01 ), TNFα and FFA were possively correlated with Homa - IR significantly ( r=0. 646,0. 601, respectively, all P 〈0.01 ) , and negatively correlated with HBCI ( r = - 0. 245, -0. 244, respectively, all P 〈 0. 05 ). Conclusion TNFα and FFA concentrations are increased in subjects with IGT and T2DM and they are all correlated positively with Homa - IR significantly and negatively with HBCI, suggesting that elevated serum TNFα and/or FFA may directly contribute to the underlying pathophysiology of type 2 diabetes, including the development of insulin resistance andβeell dysfunction.
