摘要 目的:探讨颅内动脉瘤手术的麻醉处理原则。方法:分析34例颅内动脉瘤麻醉方案,术中施行控制性降压及脑保护等措施预防术中脑动脉瘤破裂及术后脑血管痉挛,术中监测ECG、MAP、HR、NIBP、SPO,比较诱导期前后HR、MAP的变化。结果:麻醉及降压效果满意,无手术死亡病例,术中动脉瘤破裂1例。诱导期全组HR与诱导前相比无统计学意义(P>0.05),MAP与诱导后比较差异有显著性降低(P<0.05)。结论:麻醉药物及控制性降压药物选择原则要考虑对脑局部的影响,围手术期进行脑保护可以降低术后动脉痉挛和脑梗死发生率。 Objective :To investigate the principle of anesthetic management for intracranial aneurysm. Method:The anesthetic regi- mens of 34 cases of intracranial aneurysm were analyzed. Controlled lowering of blood pressure and protective measures for brain were implemented introperatively to prevent rapture of cerebral aneurysm and postoperative cerebrovascular spasm. Intraoperative ECG, MAP, HR, NIt3P and SPO2 were monitored and the MR, MAP before and after induction phase were compare. Result :The effects of anesthesia and lowering of 13p were satisfactory, no death occurred intraoperativety,and aneurysm ruptured m one case. The H- Rs of whole group during induction phase were not decreased as compared with those before induction phase, and the decrease of MAP was extremely significant as compared with that after induction phase. Conclusion :The effect on parts of brain should be considered when choosing narcotics and hypotensors. Perioperative brian protection could reduce the postoperative incidence rates of arterial spasm and cerebral infarction.
