摘要
【目的】探讨PCI时球囊扩张释放支架对心电图QRS时限及QRS额面电轴的影响。【方法】130例冠心病患者,观察球囊扩张释放支架前后QRS时限及QRS额面电轴的变化。【结果】左前降支球囊扩张释放支架后,QRS波时限延长(81.5±11.7 ms比94.0±12.9 ms,P<0.01)、QRS额面电轴左偏[(47.8±22.6)度比(26.0±34.8)度,P<0.01];右冠状动脉球囊扩张释放支架后,QRS时限延长[(81.5±11.40)ms比(93.7±12.6)ms,P<0.01]、QRS额面电轴右偏[(47.6±23.0)度比(64.1±24.6)度,P<0.01];左回旋支球囊扩张释放支架后QRS时限延长[(83.9±11.7)ms比(93.8±12.3)ms,P<0.01],对于左冠优势型的患者QRS额面电轴右偏[(48.7±20.9)度比(64.2±19.3)度,P<0.01],对于右冠优势型的患者QRS额面电轴左偏[(51.2±24.7)度比(30.6±25.4)度,P<0.01]。【结论】球囊扩张释放支架后,QRS时限延长及QRS额面电轴的变化可能是急性心肌缺血的标志。
[Objective]To investigate the effects of balloon inflation to implantation stent on QRS duration and frontal plane QRS axis during percutaneous coronary intervention. [Methods]QRS duration and frontal plane QRS axis were measured before and after balloon inflation to implantation stent in 130 patients with coronary artery disease during percutaneous coronary intervention. [Results]QRS duration was prolonged [from (81.5±11.7)ms to (94.0±12.9) ms, P 〈0.01] and frontal plane QRS axis was shifted to the left [from (47.8_±22.6) degrees to (26.0±34.8) degrees, P〈0.01] after balloon inflation to implantation stent in left anterior descending artery; QRS duration was prolonged [from (81.5±11.4) ms to (93.7±12.6) ms, P〈 0.05] and frontal plane QRS axis was shifted to the right [from (47.6±23.0) degrees to (64.1±24.6) degrees, P〈0.01] ,after balloon inflation to implantation stent in right coronary artery; QRS duration was prolonged [from (83.9±11.7) ms to (93.8±12.3) ms, P 〈0.05] and frontal plane QRS axis was shifted to the right [from (48.7±20.9) degrees to (64.2± 19.3) degrees, P 〈0.01] for patients with the dominant left coronary artery and to the left [from (51.2±24.7) degrees to (30.6±25.4) degrees, P 〈0.01] for the patients with the dominant right coronary artery after balloon inflation to implantation stent in left circumflex artery. [Conclusion]QRS prolongation and frontal plane QRS axis deviation after balloon inflation to implantation stent in coronary artery may be a marker of myocardial ischemia.
