摘要
目的观察单侧三维外固定架治疗胫骨中下段骨折的疗效。方法55例胫骨中下段骨折患者,采用单侧三维外固定架治疗,对于长斜型、螺旋型及粉碎型骨折加用螺钉作有限内固定,并与既往该院所行内固定患者作疗效对比。结果随访6～24个月,平均(13.22±5.72)个月,外固定治疗组骨折达到解剖愈合者39例,功能愈合9例,延迟愈合者2例,不愈合者3例,浅表感染2例,骨折愈合时间为2￣6个月。结论单侧三维外固定架具有操作简便、损伤少、固定可靠、有效促进骨折愈合等特点,临床上值得推广。
[Objective] To explore the effect of unilateral three dimensional external fixator in the treament of distal tibial fracture. [Method] 55 cases of distal tibial fracture were treated with unilateral three dimensional external fixator under C-arm monitor, the fracture healing and complications were observed and compared with the eorrespondant data of internal fixation group which were treated with compression plate or intrameddullary nail. [Result] All cases were followed for 6-24 months （13.22±5.72）. The delayed union rate was 2/55 in group of external fixation and 4/38 in group of interal fixation; the nonunion rate was 3/55 and 4/38; the rate of infection was 2/55 and 3/38. The difference was not significant （P =0.082）. [Conclusion] Unilateral three dimensional external fixator is a safe and effective teehique in the treament of distal tibial fracture, it has advantages of short time of operation, minimum tissue trauma, few complication, stabe fixation and is worthy of wide application in clinic.
