摘要
目的观察血清瘦素在慢性阻塞性肺病(chronic obstructive pulmonary disease,COPD)患者营养不良发生中的作用,及其与肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)的关系。方法本试验纳入患者均为男性。47例COPD患者分为非营养不良组(COPDⅠ组)29例,营养不良组(COPDⅡ组)18例。正常对照组34例。测定体重指数(body mass index,BMI)、理想体重百分比(percent ideal body weight,IBW%)、三头肌皮褶厚度(triceps skin-foldthickness,TSF)、上臂围(mid-upper arm circumference,MAC)、上臂肌围(mid-upper arm muscle circumference,MAMC)、血清瘦素、TNF-α浓度、血清前白蛋白(serum prealbumin,PA)、血清转铁蛋白(serum transferring,TF)、血清白蛋白(serum albumin,Alb)、总淋巴细胞计数(total lymphocytes count,TLC)、第1秒用力呼气容积占预计值百分比(forced expiratory volume in one second,FEV1)、最大口腔吸气压(maximal inspiration pressure,MIP)、最大口腔呼气压(maximal expiration pressure,MEP)等指标。分析各组间各项指标的差异及相关性。瘦素测定用放射免疫法,TNF-α测定用ELISA法。结果(1)COPDⅡ组血清瘦素的质量浓度[(4.07±3.42)μg/L]明显低于COPDⅠ组[(9.72±6.67)μg/L]及正常对照组[(8.21±5.41)μg/L],差异有统计学意义(P<0.05)。(2)COPDⅡ组血清TNF-α的质量浓度[(8.03±3.37)ng/L]与COPDⅠ组[(7.25±2.08)ng/L]相比差异无统计学意义。(3)各组血清瘦素浓度与TNF-α均无显著相关性。结论在合并营养不良的COPD患者中,血清瘦素仍维持正常的生理调节。在COPD患者的能量代谢调节过程中,血清TNF-α与血清瘦素之间不存在相关性。
Objective: To explore the function of serum leptin in COPD patients with malnutrition, and to investigate the relationship between leptin and TNF-α. Methods: A total of 81 subjects (47 COPD patients and 34 control subjects) participated in this study. The 47 COPD patients were divided into 2 groups: group COPD Ⅰ (patients without malnutrition during stable disease, n = 29), group COPD Ⅱ (patients with malnutrition during stable disease, n = 18). To eliminate the effect of sex differences, all the patients and controls were male. Body mass index ( BMI), percent ideal body weight ( IBW% ), triceps skin-fold thickness (TSF) , mid-upper arm circumference (MAC) , mid-upper arm muscle circumference(MAMC) ,serum leptin and tumor necrosis factor-α (TNF-α) levels, serum prealbumin ( PA), serum transferrin ( TF), serum albumin (Alb) , total lymphocytes count (TLC) , forced expiratory volume in one second (FEV~), maximal inspiration pressure( MIP)and maximal expiration pressure(MEP) were measured in all participants. Leptin levels were measured by radioimmunoassay. TNF-α levels were measured by ELISA. The between group difference and correlation of these parameters were analysed. Results: (1)Serum leptin levels were significantly lower in group COPD Ⅱ (4.07 ±3.42 μg/L) than in group COPD Ⅰ (9.72±6.67μg/L)and controls(8.21 ±5.41μg/L, P〈0.05). (2)There were no statistical differences in serum TNF-α levels between group COPD Ⅲ (8.03± 3.37 ng/L) and group COPD Ⅰ (7.25 ±2.08 ng/L). (3)There was no significant correlation between leptin and TNF-α in any group. Conclusion: Leptin was not involved in anorexia and weight loss of COPD patients. There was no significant correlation between TNF-α and leptin during the regulation of the energy balance in COPD patients.
