摘要
目的探讨适应性支持通气(ASV)模式在稳定期慢性阻塞性肺疾病(COPD)患者中的临床应用。方法选择2004年1月~2005年3月老年病房长期行机械通气治疗的稳定期COPD患者11例,分别心用三种呼吸模式,容量-辅助控制通气(CMV1,潮气量7~9mL·kg^-1·min^-1,呼吸频率16~18次/min)、ASV、CMV2(潮气越和呼吸频率与应用ASV模式时相同)。应用每种呼吸模式后,均待患者呼吸、心率平稳30min后,记录其呼吸力学指标:潮气量、呼吸频率、气道峰压、平均气道压、吸气流速、呼气流速、吸气阻力、呼气阻力、吸气时间常数及呼气时间常数等,同时测定心率和血压,抽取动脉血行血气分析:结果11例COPD患者应用ASV模式,与CMV1模式比较,潮气量明显升高,呼吸频率明显减慢,吸气流速、吸气时间常数及呼气时间常数均升高(P〈0.05),呼气流速下降(P〈0.05)。气道峰压、平均气道压、吸气阻力和呼气阻力也有所下降,但无统计学意义(P〉0.05)。应用两种呼吸模式时心率、平均动脉压及pH、PaCO2、PaO2比较差异均无统计学意义(P〉0.05)。11例患者从ASV模式改用CMV2模式后,气道峰压及平均气道压有所升高(P〉0.05),心率、平均动脉压及pH、PaCO2、PaO2比较差异亦无统计学意义(P〉0.05)。应用ASV模式时,患者舒适程度好。结论稳定期COPD患者应用ASV模式,可减少患者的呼吸用力,人-机协调性和安全性良好,在临床上可推广应用。
Objective To study the clinical application of adaptive support ventilation on stable chronic obstructive pulmonary disease(COPD) patients with long-term ventilation.Methods 11 stable elderly COPD patients with long-term ventilation were enrolled from Jan 2004 to Mar 2005. Three modes with the same minute volume were applied,ie,CMV1 with a tidal volume of 7- 9 mL·kg^-1· min^-1 and respiratory rate of 16- 18/min, ASV, CMV2 (with the stone parameters as ASV). The variables of respiratory mechanics were recorded. The variables of hemodynamics and arterial blood analysis were measured 30 min after stable breathing condition in all modes. Results Compared with CMV1, tidal volume increased and respirator, rate decreased in ASV with increased inspiratory flow, inspiratory time constants and expiratory time constants( P 〈 0.05 )and decreased expiratory flow( P 〈 0.05),but airway freak pressure, mean airway pressure, inspiratory resistance and expiratory resistance remained stable( P 〉 0.05 ). ASV had the similar impacts on respiratory mechanics, heine, dynamics and arterial blood analysis with CMV2( P 〉 0.05). However, the patients applied with ASV felt more comfortable. Conclusion ASV can be applied in the patients with stable COPD with less respiratory effort and better patientventilator interaction.
