摘要
目的为尺侧腕伸肌腱部分移位修复桡尺远侧关节脱位提供解剖学依据以及临床资料。方法30侧成人上肢标本,将尺侧腕伸肌腱分为上、中、下3部分,进行形态学测量和力学测试。结果力学测试:尺侧腕伸肌全肌腱载荷为(300.0±58.0)N,半肌腱载荷为(155.8±33.7)N,两者平均破坏荷载差别无统计学意义。2种术式设计:(1)环状韧带重建法;(2)钻孔固定法。临床应用12例,均获得良好效果。结论尺侧腕伸肌腱部分转位有足够的长度和强度以修复桡尺远侧关节脱位,术式设计合理。
Objective To provide anatomical basis and clinical data for the operation of the transposition of partial tendon of extensor carpi ulnaris to repair dislocation of the distal radio-ulnar joint. Methods Tendon of extensor carpi ulnaris divided into three parts(proximal, middle and distal) for survey the morphologic configuration and mechanics strength test of tendon of flexor carpi ulnaris on 30 adults cadaver specimens. Results Mechanics test: the strength of integrity tendon of extensor carpi ulnaris was (266.7 ±45.2)N, the half one was (107.2 ±19.9)N. The strength of integrity and half tendon of extensor carpi ulnaris had no significant difference. Two operations were designed: (1)reconstruction of annular ligament; (2)drilling fixation. Satisfactory results had been achieved in 12 cases. Conclusion The transpose of partial extensor carpi ulnaris tendon has enough length and intensity for repair dislocation of the distal radio-ulnar joint.
