脑弥漫性轴索损伤166例临床分析 被引量:6

摘要 目的探讨脑弥漫性轴索损伤(DAI)的临床特征、诊断要点和治疗方案。方法回顾性分析166例DAI的临床和影像学资料。结果(1)166例DAI患者,存活92例(55.42%),其中56例恢复良好,23例中残,10例重残,植物生存3例;死亡74例(44.58%)。(2)DAI的临床特征头部暴力损伤病史;伤后即刻昏迷,且持续时间长;多数患者临床症状重,但无明确定位神经系统体征。(3)DAI的诊断要点头部外伤史、临床特征和影像学表现。(4)DAI的治疗以保持呼吸道通畅、控制颅内压、降低脑代谢及防治各种并发症为主要措施。结论DAI是影响颅脑损伤病死率、致残率的最重要因素,正确认识其临床特征,掌握诊断要点,有效地采取综合的治疗措施,是提高颅脑损伤救治成功率的关键。 Objective To investigate the clinical feature,diagnostic main points and treatment of diffuse axonal injury(DAI). Methods The dinical materials and imaging findings of 166 cases with DA1 were analyzed retrospectively. Resuits ( 1 )The surviors were 92 cases(55, 42 % ), 56 cases showed a gczxl outcome, while 23 were moderatdy disabled, 10 severdy disabled, 7 survived in a permanent vegetative state; 74 died, the mortality rate was 44, 58 %. (2)The clinical feature of DAI : medical history of vidence in head; Immediate coma after injury; No local neurological sign. (3) Diagnostic main points of DAI:external injury of head,clinical feature and imaging finding. (4)Treatment of DAI:to preserve the fluent respiratory tract, to control intracranial pressure, to decrease brain metabolism and to prevent every complication were principal ways, Conclusions DAI is the mint important factor that influence fatality rate and mutilation rate of brain injury. Comprehenaive diagnosis and therapy are crucial in clinical practice.
