甘露醇对脑出血早期血肿扩大影响的动物实验研究 被引量:30

The mannitol effect on early enlargement of hematoma in intra-cerebral hemorrhage animals

摘要 目的探讨甘露醇对脑出血(ICH)早期血肿扩大的影响。方法选择在DSA介入下制作、血肿体积≤4ml的犬ICH模型30只,随机分为甘露醇组和对照组,每组各15只,于24h和48h分别复查脑CT,血肿体积增大≥1/3为早期血肿扩大,分析早期应用甘露醇对血肿扩大的影响。结果甘露醇组48h内发生血肿扩大者明显多于对照组(P<0.05),且前者24h内发生早期血肿扩大者明显多于自身24~48h者(P<0.05);甘露醇组48h内死亡3只,对照组无1只死亡。结论早期使用甘露醇可增加犬ICH早期扩大的发生率;ICH发病早期,尤其是24h内不宜盲目使用甘露醇,以免诱发血肿扩大,加重病情。 Objective To explore the effect of mannitol on early enlargement of hematoma in intra-cerebral hemorrhage. Methods The middle cerebralarteries of 30 healthy domestic dogs were pricked with DSA and dog model of ICH with a hematoma≤4 ml in volume was prepared. They were divided randomly into two groups by given or not given mannitol. Head CT scans were examined cat both 24 and 48 hours after hematoma appearance. Hematoma increased by 1/3 in volume was defined early enlargement of hematoma and effect of mannitol on early enlargement of hematoma and effect of mannitol on early enlargement of hematoma was analyzed. Results Hematoma enlargement in mannitol group within 48h was more common than that in nonmannital group (P〈0. 05) ; In the former hematoma enlargement was more common within 24h than that during 24~48h (P〈0. 05), Three dogs died in mannitol group within 48h, butno dog died in control group. Conclusions Early application of mannitol may increase the rate of hematoma enlargement in intra-cerebral hemorrhage of dog; In early intra-cerebral hemorrhage, especially within 24h, mannital should not be used blindly because of inducing hematoma enlargement and aggravating state of an illness.
