幽门螺杆菌感染与食管鳞癌的相关性研究 被引量:1

摘要 目的:探讨幽门螺杆菌(Helicobacter pylori,H.pylori)感染与食管鳞癌发生的相关性。方法:应用Giemsa染色检测185例食管鳞癌手术标本和60例对照组(食管鳞癌手术切缘正常组织)、80例慢性食管炎、50例食管非典型增生中的H.prlori检出率;同时用免疫组化染色法检测食管鳞癌组织、癌旁组织、远癌组织中H.pylori蛋白表达。结果:Giemsa染色法,H.pylori检出率在正常食管黏膜组、慢性食管炎组、食管非典型增生组和食管鳞癌组分别为21.67%、38.75%、46.00%和65.93%,差异有统计学意义(P<0.05);H.pylori检出率在食管鳞癌组织、癌旁组织和远癌组织分别为65.93%、37.70%和22.65%(P<0.05);免疫组织化学染色法,食管鳞癌手术切除标本癌组织、癌旁组织及远癌组织H.pylori阳性率分别为76.92%、48.63%和30.94%,提示H.pylori检出率在食管鳞癌不同部位间差异有统计学意义(P<0.05)。在食管鳞癌组织,Giemsa染色法,汉族、哈萨克族和维吾尔族H.pylori检出率分别为63.53%、70.59%和66.67%;免疫组织化学染色法,汉族、哈萨克族和维吾尔族H.pylori检出率分别为74.12%、84.31%和75.56%,提示汉族、哈萨克族和维吾尔族三民族之间H.pylori检出率差异无统计学意义(P> 0.05)。结论:H.pylori感染与食管鳞癌发生存在相关性;食管鳞癌H.pylori感染主要位于癌组织及癌旁组织,远癌组织则少见;汉族、哈萨克族、维吾尔族H.pylori检出率无差异;提不哈萨克族食管鳞癌高发,除H.pylori感染,可能还有其他因素共同参与了食管鳞癌的发生、发展。
