摘要 目的 介绍长期留置血液透析(下称血透)导管的置入方法、透析效果以及并发症。方法 采取带皮下隧道长期留置导管置入术建立血透通路15例,观察其透析效果与并发症的发生情况。结果 15根长期留置血透导管共使用2820d,平均每根留置时间188d,其透析效果优于人造血管和临时性血透导管,但不及动静脉内瘘。长期留置血透导管常见的并发症为血流不畅和感染。经干预均可控制。结论 长期留置血透导管对于老年人、心功能不全或造瘘困难者是较好的选择,其透析效果较好,并发症较少。 Objective To recommend the placement, performance and complications of long term venous indwelling catheter. Method In recent years, we used long-term venous indwelling catheter placed in the right internal jugular veins or the right jugular veins for hemodialysis in 15 cases. Adequacy of hemodialysis by long-term venous indwelling catheter had been compared with other vascular access. Common complications of long term venous indwelling catheter and it's prevention and care'had also been discussed. Results Up to the present, 15 catheters were placed for a total number of 2820 catheter-days. Hemodialysis adequacy of long-term venous indwelling catheter was better than artificial blood vessel and temporary hemodialysis catheter, but wasn't so good as arteriovenous fistula (AVF). The predominant complications were catheter thrombosis, catheter fibrin sheathing and infection, and most of them can be controlled after intervention. Conclusions For those who are not suitable candidates for any other mode of hemodialysis access, such as elderly, heart failure, and those who had a history of one or more previously failed arteriovenous accesses, long-term venous indwelling catheter is a alternative vascular access with few complications and adequate dialysis.
