摘要
目的观察10%葡萄糖酸钙脐静脉注射对妊娠高血压综合征(妊高征)患者术中、产后出血的影响。方法观察组85例,在胎儿娩出后立即断脐,经脐静脉向母体方向注射10%葡萄糖酸钙10ml;对照组87例,胎儿娩出后立即子宫切口缘注射缩宫素20U。观察二组患者第三产程时间、术中出血量、产后出血发生率以及观察组用药前后血压的变化。结果①二组患者的年龄、孕周、产次、轻中重度妊高征所占的比例,产前及产时处理、新生儿体重相比,差异无统计学意义(P>0.05)。②观察组,第三产程时间(7.16±3.23)min,术中出血量(176.42±93.20)ml,产后出血发生率3.53%(3/85);对照组,第三产程时间(9.35±3.67)min,术中出血量(281.23±122.33)ml,产后出血发生率9.20%(8/87),二组比较差异均有统计学意义(P<0.05)。③观察组用药前收缩压(153.0±10.6)mmHg,舒张压(96.0±10.3)mmHg,用药后收缩压(136.0±10.4)mmHg,舒张压(80.0±10.2)mmHg,用药前后收缩压及舒张压比较差异均有统计学意义(P<0.05)。结论妊高症患者术中胎儿娩出后经脐静脉注射葡萄糖酸钙预防术中、产后出血及控制血压效果良好。
Objective To observe the effect of calcium gluconate injected into umbilical vein on operative bleeding of cesarear, section in patients with pregnancy induced hypertension(PIH). Methods 85 patients in observe group injected 10% calcium gluconate 10ml by umbilical veins into maternal side of placenta, while 87 patients in control were given oxytocin 20U by injecting uterine body after fetus delivered and cord cut off. We observed time of the third stage of labor, rate and amount of operative bleeding as well as change of bleed pressure premanagement and promanagement of calcium glueonatc. Results ① Age, gestational age, times of lying, degree of PIH, measure of prelabour and inlabor and newborns weights had no significant different between the two groups (P 〉 0.05). ② Time of the third delivery stage, rate and amount of bleeding were (7.16±3.23) min, 3.53% (3/85), (176.42±93.20) ml respectively in observe group. They were obviously decreased when compared with those in control (P〈0.05). ③ In observe group, the systolic and diastolic pressure before injection were (20.4±2.11)kPa and (12.8±1.84)kPa, (18.2±1.89)kPa and (10.6±1.64)kPa after injection. The difference between them was significant (P〈0.05). ④ The side effect in observe group appeared only in 1 patients, who showed face rubeosis and instantaneous beat, but it needn't be managed. Conclusion It is a good precausion against operative and postpartum bleeding to inject calcium gluconate by umbilical vein in cesarean patients with PIH.
