摘要
目的探讨经皮穿刺半月节及外周支射频控温热凝治疗三叉神经痛的疗效。方法本组三叉神经痛患者407例,分别经半月节及外周支射频控温热凝治疗。结果半月节射频控温热凝316例,一次治疗完全止痛298例(94.3%),一周后第二次治疗完全止痛18例(5.7%)。随访1￣9年,复发145例,经再次治疗后随访1￣8年又复发53例;该53例经第三次治疗随访至今未见复发。外周支射频控温热凝91例,一次治疗后均完全止痛。随访6年,复发66例,行第二次热凝治疗后随访1￣5年再次复发31例;该31例经第三次治疗后,随访1￣4年再次复发5例;该5例经第四次治疗后至今未复发。结论半月节及外周支射频控温热凝治疗三叉神经痛,能保留触觉仅破坏痛觉,止痛效果确切,相对安全,适应证广,操作较简便,可重复进行。
Objective To explore the curative effects of percutaneous radiofrequency lesion of the semilunar ganglion and it＇s peripheral branches on the trigeminal neuralgia. Methods Of 407 patients with trigeminal neuralgia, 316 were treated by radiofrequency lesion of the semilunar ganglia and 91 by radiofrequency lesion of peripheral branches corresponding to trigeminal neuralgia. Results In 316 patients the treated by the lesion of the semilunar ganglia, the neuralgia was completely controlled by one lesion in 298 patients, and the neuralgia was completely controlled by the second lesion one week after the first lesion in 18 patients. The following up from 1 to 9 years showed that the trigeminal neuralgia recurred in 145 patients, who were retreated by radiofrequency lesion. The neuralgia recurred again in 53 of 145 patients 1-8 years after the second treatment. Up to now the neuralgia does not recur in these 53 patients after the third lesion. The neuralgia was compeletly controlled by one treatment in all the 91 patients undergoing the lesion of the peripheral branches. The following up for 6 years showed that the neuralgia recurred in 66 of the 91 patients. The neuralgia recurred again in 31 of the 66 patients 1-5 years after the second lesion. The third radiofrequency lesion was performed in the 31 patients, of whom, 5 suffered again from trigeminal neuralgia 1-4 years after the third lesion. Up to now the neuralgia does not recur in the 5 patients after the fourth lesion. Conclusions Radiofrequency lesion of the semilunar ganglion and it＇s peripheral branches for the treatment of trigeminal neuralgia can damage only to a sense of pain and remain a sense of touch, has reliable analygsic effect on trigeminal neuralgia, is easily manipulated and relatively safe, and can be repeatedly performed.
