摘要
目的:探讨不同时间介入“醒脑开窍”针刺法对脑梗死患者的临床疗效及神经功能缺损动态改变的影响。方法:选符合标准的该病住院患者120例,依据病程分为4组:组(超早期)、组(7h^3天)、期(4~7天)、组(21~82天)。分别观察“醒脑开窍”针刺法对各组患者的临床疗效及治疗前、后7、14、21天的神经功能缺损评分的参数变化。结果:组总有效率为96.67%(95%CI=82.8%~99.9%),、组总有效率均为93.33%(95%CI=82.2%~99.9%),组的总有效率为66.67%(95%CI=47.2%~82.7%);组间综合疗效比较,有明显差异(χ2=36.3517,P<0.005);且组与组总有效率的95%CI域值不重叠。治疗后第7、14、21天时患者的神经功能缺损减少以组最佳;组间比较均有明显差异(P<0.05)。结论:“醒脑开窍”针刺法针刺治疗对不同病变时期的患者均有一定疗效,尤其是超早期介入,更有助于提高临床疗效及促进脑梗死患者神经功能缺损的恢复,其收益明显。
Objective： To investigate and observe the clinical effect of Xingnao Kaiqiao Acupuncture（XKA） therapy on neurological impairment in different stages after cerebral infarction. Methods ：The in-patients with cerebral infarction in different stages were divided into four groups：group Ⅰ （acute stage） ,group Ⅱ （7^thhour to 3^rdday）,group Ⅰ （4^rdday to 7^thday）,group Ⅳ （21^stday to 82^ndday）. The changes of the neurological impairment scores and the clinical effect of the four groups were observed before intervention and on the 7^th day,the 14^th day,and 21^st day after therapy. Result ：The best effect was observed in group Ⅰon the 7^thday,the 14^thday and 21^stday after therapy. The difference between group Ⅳ and group Ⅰ ,group Ⅱ and group Ⅲ were significant （P 〈 0. 05）. Conclusion：There was good effect of XKA on patients in different stages,especially in acute stage.
