胰腺自身免疫抗体对2型糖尿病患者胰岛β细胞功能的影响 被引量:2

Influence of islet autoimmune antibodies on islet beta-cell function of type 2 diabetic patients

摘要 目的探讨胰岛细胞抗体(ICA)及谷氨酸脱羧酶抗体(GAD)对2型糖尿病患者胰岛素分泌第一时相及第二时相的影响.方法对315例初诊为2型糖尿病的患者行静脉葡萄糖耐量试验(IVGTT),测定0、1、2、3、10、30、60、120、180分钟血糖、胰岛素及其他临床指标.结果 ICA阴性和阳性组患者比较,第一时相胰岛素分泌在抗体阳性组明显下降,表现为阴性组比阳性组的IVGTT 0~10分钟曲线下面积[AUC(0-10)]为(119.48±222.02)mU×min/L vs(69.46±71.26)mU×min/L(P=0.006),第一时相胰岛素峰值平均数(LN值)为(1.96±0.96)mU/L vs(1.46±0.96)mU/L(P=0.003);GAD阴性和阳性组患者比较,第一时相胰岛素分泌在抗体阳性组明显下降,表现为第一时相胰岛素峰值平均数(LN值)为(1.97±0.97)mU/L vs(1.69±0.96)mU/L(P=0.036),AUC(0-10)为(123.70±237.00)mU×min/L vs(81.13±74.44)mU×min/L(P=0.017);任意一抗体为阴性患者与ICA和GAD抗体均为阳性患者比较,第一时相胰岛素分泌在抗体阳性组明显下降,表现为第一时相胰岛素峰值平均数(LN值)为(1.92±0.98)mU/L vs(1.30±0.77)mU/L(P=0.017),AUC(0-10)为(115.89±213.63)mU×min/L vs(49.10±29.33)mU×min/L(P=0.000).结论 ICA和GAD抗体是影响2型糖尿病患者胰岛素分泌第一时相的重要因素. Objective To investigate the influence of glutamic acid decarboxylase antibody(GAD-Ab) and islet cell antibody(ICA) on the first and second phases of insulin secretion in type 2 diabetic patients. Methods The intravenous glucose tolerance tests(IVGTT) were measured on 315 first-diagnosed type 2 diabetic patients,and at 0,1, 2,3,10,30,60,120,180 minutes,the blood glucose,insulin level and others were noted down. Results Compared with ICA negative patients,ICA positive patients showed lower first phase insulin secretion,area under curves(AUC)(0-10min) was (119.48±222.02) mU× min/L vs (69.46±71.26) mU× min/L( P =0. 006) ,the average insulin level of the first phase was (1.96±0. 96) mU/L vs (1.46±0. 96) mU/L( P =0. 003) ; compared with GAD negative patients,GAD positive patients also showed lower first phase insulin secretion, the average insulin level of the first phase(1.97 ± 0. 97) mU/L vs (1.69±0. 96) mU/L( P = 0. 036), AUC(0-10min) was ( 123. 70 ± 237.00) mU × min/L vs (81.13 ± 74.44) mU×min/L( P = 0.017);and both ICA and GAD antibodies positve patients showed lower first phase insulin secretion,AUC(0-10min) was (115.89±213.63) mU· min/L vs (49.10±29.33) mU×min/L( P =0. 000) ,the average insulin level of the first phase was (1.92±0. 98) mU/L vs (1.30±0. 77) mU/L( P = 0. 017). Conclusion ICA or GAD antibodies are important factors effecting on the first phase of insulin secretion.
