舒芬太尼与芬太尼用于术后静脉自控镇痛的比较 被引量:3

摘要 目的以等效价剂量芬太尼为对照,探讨舒芬太尼用于术后静脉自控镇痛的优越性。方法60例择期全麻下开腹手术患者,ASAI-Ⅱ,随机分为舒芬太尼(SF)组和芬太尼(F)组,每组30例。SF组术后镇痛以舒芬太尼3μg/kg,F组术后镇痛以芬太尼0.024mg/kg,均稀释至100ml,负荷量3ml,背景剂量2ml/h,自控追加剂量2ml/次,锁时15min。观察病人术后4h、12h、24h、40h疼痛评分、镇静评分和舒适程度评分、脉搏氧饱和度、PCA有效按压情况,以及恶心、呕吐的发生情况。结果术后4h、12h、24h舒芬太尼组的疼痛评分明显低于芬太尼组(P<0.05)。术后4h、12h的镇静评分、舒适评分舒芬太尼组高于芬太尼组。术后4h、12h、24h时点间舒芬太尼组的按压次数明显少于芬太尼组。SPO2在两组间无明显差别。术后40h内舒芬太尼组恶心、呕吐发生率明显低于芬太尼组(SF:5/30,F:12/30;P<0.05)。结论舒芬太尼是一较好的术后静脉镇痛药物,较芬太尼有着较强的镇痛、镇静作用,患者舒适程度较好,副作用少。 Objective To investigate the effect of sufentanil in the postoperative patient controlled intravenous analgesia (PClA)eomparison with equipotent doses of fentanyl. Methods 60 patients, ASAⅠ-Ⅱ ,scheduled for abdominal surgery under general anesthesia, were randomly assigned into two groups(n=30 per group): Sufentanil(SF) and Fentanyl(F).Patients in group SF reeevied sufentanil as postoperative analgesia. The dose of sufentanil was 3μg/kg and diluted with saline water for 100ml. The PCIA setting: loading dose 3 ml, bolus of 2 ml with lock time 15 minutes and background dose 2ml/h. Fentanyl 0.024mg/kg was a substitue for sufentanil in group F. Scores of analgesia, sedation and comfort were evaluated,and recorded the SPO2, times of PCA key pressed , nausea and vomiting at 4h,12h,24h and 40h postoperatively. Results The scores of analgesia in group SF were lower significantly than that in group F at 4h,12h,24h postoperatively. And the scores of sedation and comfort in SF were higher markedly than that in group F at 4h,12h after surgery. The incidence of nausea and vomiting were lower in group SF. Conclusion Sufentanil is better for postoperative patient controlled intravenous analgesia compare with fentanyl.
