摘要 目的探讨肾移植患者外周血中肿瘤坏死因子(TNF-α)与转化生长因子(TGF-β)基因多态性与急性排斥反应的关系。方法应用聚合酶链反应测定90例肾移植患者外周血中TNF-α、TGF-β的基因型,分析其基因型的分布频率以及不同细胞因子基因型对肾移植效果的影响。结果TNF-α高分泌型的肾移植受者,移植术后急性排斥反应发生率显著高于低基因分泌型受者(P<0.05);TGF-β的基因表达未能显示与急性排斥反应的相关性。结论细胞因子TNFα的基因型多态性对肾脏移植后急性排斥反应发生率有明显的影响,提示移植后免疫反应的强度与遗传学相关。, [Objective] To analyze the relation between selected cytokine tumor necrosis faetor-α (TNF-α) and transforming growth factor-β(TGF-β) genotypes in kidney recipients with acute rejections. [Methods] TGF-βand TNF-α genes polymorphisms of peripheral blood were determined using DNA polymerase chain reaction technology in 90 kidney transplant recipients (1999 to 2004). The patients were followed up for 4 to 30 months. According to the distribution of alleles of selected genes, the occurrence of acute rejection was compared among the groups. [Resuits] The Kidney recipients with TNF-α high producer genotype were correlation with more acute rejections episodes than those with lower producer genotype. TGF-β gene polymorphisms had no effect on the incidence of acute rejections. [Conclusion] The TNF-α gene polymorphism has significant influence on incidence of acute rejection after renal transplantation. The data suggest that the strength of the alloimmune response after transplantation is in part genetically determined.
