摘要
目的调查类风湿性关节炎（RA）治疗前的生活质量。方法对48侧患者采用自己填写生活质量量表及医生与患者交谈的方式进行调查。调查生活质量量表内容包括生理功能、社会功能、心理功能、健康自我认识四个方面。结果45．3％RA患者的生活质量受到严重影响，其中43．4％RA患者生理功能、53．7％RA患者社会功能、51．5％RA患者心理功能、32．7％RA患者健康自我认识受到严重影响。不同年龄段患者生活质量比较发现，20岁～、40岁～患者心理功能平均得分明显高于60岁～患者（P〈0．05）。40岁～患者健康自我认识平均得分明显高于20岁～和60岁～患者（P〈0．05）。不同病程RA患者生活质量比较发现，0～5年病程比10年以上患者生理功能平均得分明显降低（P〈0．05）。相关分析发现，生活质量的生理功能方面与社会功能、健康自我认识能力呈直线正相关，心理功能与健康自我认识能力呈直线正相关。结论近半数RA患者生活质量受到严重影响，其中心理因素受严重影响比例最高，生活质量下降与患者的年龄、病程密切相关，提示RA患者宜早期治疗，尤其注重调整好中青年RA患者心理状态。
Objective To investigate and analyse the quality of life in patients with rheumatoid arthritis （RA） untreated. Methed 48 patients were investigated by direct questioning using an uniform questionnaire. The graph of the quality of life included physical function, social function, emotional function, self-recognized health status. Results life quality of 45.3% RA patient, which physical function had 43. 4%, which social function had 53.7%, which emotional function had 51.5%,which self-recognized health status had 32. 7%, were seriously affected. By comparison, the average scores of the varied age RA patients, from 20 to 59, in emotional function, was higher significantly than patients over 60 （P〈0. 05）. the average scores of RA patients, from 40 to 59, in self-recognized health status ,was higher significantly than the others, from 20 to 39 and over 60（P〈0. 05）. the varied history patients , we can find that the average scores of RA patients , from 0 to 5 year, was higher significantly than the others , over 10 years（P〈0. 05）. Physical function was positively related to social function and self-recognized health status in RA patients. Emotional function was positively related to self-recognized health status in RA patients. Conclusion About fifty percent RA patients suffered from serious effects of their quality of life. The proportion of emotional factor was the higest in the four factors. The descent of their quality of life was related to their ages and disease histories. So we point out that The RA patients should be treated early, especially it is indispensable to improve emotional function in young-and middle-aged RA patients.
