摘要
目的观察非ST段抬高急性冠状动脉综合征病人N端前脑钠肽(N-terminal pro-B-type natriuretic peptide,NT—proBNP)血清浓度的改变并探讨其临床意义。方法±观察对象102例,分为4组:不稳定性心绞痛组36例,非ST段抬高心肌梗死组24例,稳定性心绞痛组20例,正常对照组22例。用电化学发光法测定NT-proBNP血清浓度并行超声心动图评价心功能。结果±不稳定性心绞痛组和非ST段抬高心肌梗死组病人的血清NT-proBNP水平明显高于稳定性心绞痛组和对照组(P<0.01);而稳定性心绞痛组与对照组之间差异无统计学意义(P>0.05);不稳定性心绞痛组和非ST段抬高心肌梗死组,NT-proBNP血清水平与左心室射血分数呈负相关(r=-0.597,P<0.01);NT—proBNP血清水平在左心室射血分数小于0.50的病人明显高于左心室射血分数正常病人(P<0.01);在左心室射血分数正常的病人中,不稳定性心绞痛组和非ST段抬高心肌梗死组病人的NT-proBNP血清水平仍明显高于稳定性心绞痛组的病人(P<0.01)。结论血清NT-proBNP 浓度在非ST段抬高急性冠状动脉综合征的病人中明显升高,升高水平与左心室射血分数明显相关。
Objectives To observe the changes of serum N-terminal pro-B-type natriuretic peptide (NT- proBNP) concentrations in patients with non-ST-elevation acute coronary syndromes, and to investigate the clinical significance. Methods 102 subjects were divided into four groups: 36 patients with unstable angina, 24 patients with non-ST-elevation acute myocardial infarction, 20 patients with stable angina, and 22 healthy subjects as control. Serum NT-proBNP concentrations were measured with automated electrochemiluminescence immunoassay and echocardiograms were detected in all patients to measure left ventricular ejection fraction (LVEF). Results (1) Serum NT-proBNP concentrations were increased in patients with unstable angina and non-ST-elevation acute myocardial infarction compared with those of stable angina and control subjects, P〈0.01, respectively. However, there was no significant difference between the patients with stable angina and control subjects, P〉0.05. (2) Serum NT-proBNP concentrations were correlated with LVEF in patients with non-ST-elevation acute coronary syndromes,r=- 0.597, P〈0.01, and increased significantly in patients with abnormal LVEF (LVEF〈0.50)compared with patients with normal LVEF(LVEF≥0.50), P〈0.01. Even though the LVEF was normal in patients with non-ST-elevation acute coronary syndromes, serum NT-proBNP levels were still higher than patients with stable angina, P〈0.01. Conclusions Serum NT-proBNP concentrations were increased significantly in patients with non-ST-elevation acute coronary syndromes; moreover, the levels of serum NT-proBNP were correlated with abnormal LVEF, which may have a clinical significance to evaluate the risk of patients with non-ST-elevation acute coronary syndromes.
