摘要
目的通过比较分析产TEM-116型β-内酰胺酶(-βlactamases)的肺炎克雷伯菌临床耐药性,了解TEM-116型β-内酰胺酶的特性。方法采用纸片扩散法进行细菌药敏试验,并以分析性等电聚焦的方法测定其等电点(isoelectric points,PI),对产TEM-116肺炎克雷伯菌与产TEM-1肺炎克雷伯菌的耐药性进行研究。结果产TEM-1和产TEM-116的肺炎克雷伯菌均对AMP 100%耐药,对第一、二代头孢菌素高度耐药,对第四代头孢菌素FEP敏感,对IPM 100%敏感;而产TEM-116肺炎克雷伯菌对AMC、TZP、AMK、GEN的耐药率较产TEM-1型者高,卡方检验显示二者之间差异存在显著性(P<0.05);分析性等电聚焦显示,TEM-116型β-内酰胺酶PI值为5.4,绝大多数产TEM-116肺炎克雷伯菌有>2条电泳带,仅1株只产TEM-116,它对大多数抗菌药物耐药。结论产TEM-116的肺炎克雷伯菌,对抗菌药物的耐受率比产TEM-1的肺炎克雷伯菌显著升高,排除其他耐药质粒的影响,TEM-116对β-内酰胺的水解能力明显高于TEM-1,具备了ESBLs的特性。
OBJECTIVE To understand characteristics of TEM-116 β-lactamases through comparative study on resistance to antibiotics of clinical isolates of Klebsiella pneumoniae producing TEM-116 β-lactamases. METHODS K. pneumoniae susceptibility to β-lactamases was determined by disk diffusion tests, and their isoelectric points (PI) were detected using analytic isoelectric focusing(IEF), and resistance to antibiotics of clinical isolates of K. pneumoniae producing TEM-116 and TEM-1β-lactamases was studied. RESULTS Both of K. pneumoniae producing TEM-116 β-lactamases and producing TEM-1 β-lactamases were 100% resistant to AMP, and highly resistant to the first and second generation cephalosporin, but greatly susceptible to FEP and IPM. There was greatly difference between resistance to AMC, TZP, AMK, and GEN of clinical isolates of K. pneumoniae producing TEM-116 β-lactamases and that of K. pneumoniae producing TEM-116 β -lactamases, the TEM-116 isolates were higher resistant than TEM-1 isolates. Analytic IEF results showed that PI of TEM-116 β-lactamases was 5.4, and most strains of K. pneumoniae TEM-116 β-1actamases displayed two electrophoresis bands or more, only one strain of them just displayed one band, resistant to majority of antibiotics. CONCLUSIONS The results show that K. pneurnoniae producing TEM-116 β-1actamases are more resistant to antibiotics than K. pneurnoniae producing TEM-1 β-lactamases, and indicate TEM-116 β-lactamases work as ESBLs.
