摘要
目的探讨甲状腺癌二维和彩色多普勒图像特点,提高早期诊断和鉴别诊断的准确性。方法对经手术病理结果证实的40例甲状腺癌患者的术前超声检查结果进行分析,重点分析其内部和边缘的回声特点、血流速度、阻力指数及血流频谱形态。结果本组病例中单发者26例,合并结节性甲状腺肿10例,合并腺瘤4例。肿瘤呈不均匀低回声31例,内显示钙化29例,出现液化5例。显示丰富血流者22例,显示棒条状血流信号16例,难以显示血流信号2例。脉冲多普勒显示平均血流速度25.2cm/s。频谱特点:峰值前移31例,阻力指数平均0.67。结论超声诊断甲状腺癌关键要抓住以下几点:①肿块内部呈低回声;②肿块边缘无包膜;③内部出现微小钙化灶;④后方回声衰减;⑤肿块血流供应丰富;⑥动脉血流呈高阻力信号;⑦血流频谱峰值前移;⑧舒张期最低流速低于收缩期峰值的1/2;⑨颈部转移性淋巴结肿大。如果出现上述中的5点,则可以考虑甲状腺癌的诊断。
Objective To discuss the two-dimensional and color Doppler ultrasonographic characteristics of thyroid carcinoma and improve the accuracy of early diagnosis and differential diagnosis. Methods Pre-operational ultrasonic characteristics of 40 patients with thyroid carcinoma which were confirmed by pathology were analyzed. The emphases were the internal and edge echo, blood flow velocity, resistance index and the spectral flow Doppler pattern. Results In all patients, 26 patients had single lump, 10 with nodular goiter, and 4 with thyroid adenoma. Thirty-one patients＂ lumps were low heterogeneous echo, 29 patients＂ had calcification, 5 patients＂ had liquescence. Twenty-two patients＂ lumps had abundant blood flow, 16 patients＂ manifested ＂streak-like＂ blood flow, 2 patients＇ had no blood signal. The pulse Doppler showed that the average blood flow velocity was 25.2 cm/s. The characteristics of spectral Doppler were： the velocity peak of 31 patients moved ahead, the average resistance index was 0.67. Conclusion When diagnosing thyroid carcinoma with ultrasound, we must emphasize： ①the internal echo of lump is lower, ②the lump has no capsule, ③microcalcification appear in the lump, ④the echo behind the lump is weakened, ⑤abundant blood flow appear in the lump, ⑥the resistance index is high, ⑦the peak of spectral Doppler moves ahead, ⑧the minimum velocity in diastole is less than the half of the peak in systole, ⑨metastatic lymph nodes at cervix are enlarge. If five points above appear, thyroid carcinoma could be diagnosed.
