点式接触动力加压接骨板固定骨微循环形态学观察的实验

摘要 目的观察点式接触动力加压接骨板(PC-DCP)和动力加压钢板(DCP)内固定后皮质骨微循环形态学的变化。方法采用性成熟山羊13只,在左右侧完整胫骨中段分别用PC-DCP和DCP固定。分别于固定术后1天、2、6、12周进行活体双硫蓝染色,固定骨段钢板床和横切面观察。结果PC-DCP实验侧术后1天在螺钉孔周围和钢板点接触部位出现皮质骨缺血,2螺钉孔中间骨段未观察到明显缺血区域,于术后2周完全恢复;DCP固定侧术后1天、2、6周,板下皮质骨外层持续存在缺血现象,直至术后12周微循环才恢复。结论PC-DCP固定可明显保护局部骨微循环。 Objective To compare the effects of point contact-dynamic compression plates (PCDCP) and dynamic compression plates (DCP) on bone microcirculation in intact goat tibia model. Methods Twelve adult goats were used in this experiment. The bilateral intact tibia of goat were fixed with DCP and PC-DCP respectively, regions of perfusion deficiency was observed 1 day, 2,6 and 12 week after the operation by Disulphine blue staining technique. Results Regions of perfusion deficiency limited to the surrounding of the drill hole site and the points where the plate contacted with the bone following PC-DCP application on the 1 st day and disappeared after 2 weeks. DCP application resulted in continuous perfusion deficiency under the plate ld,2 and 6 weeks after the operation. Conclusion PC-DCP can avoid the vascular damage under the plate compared with the conventional plates.
