摘要
目的:探讨诱导骨基质植入治疗胫骨平台骨折的临床疗效.方法:对1998年4月-2004年8月收治的胫骨平台骨折35例,男22例,女13例;年龄22~71岁,平均44.3岁,均行诱导骨基质植入治疗,全部病例术前均行正侧位X线片、CT检查,根据骨折类型和软组织损伤情况制定手术方案,术中采用“L”梯型加压钢板及拉力螺钉内固定.结果:35例获得6个月~4年,平均(28.6±8.2)个月随访.按Merchant评分标准:优19例,良10例,可4例,差2例.植骨内固定后骨折愈合时间8~15周,平均(11.3±1.6)周,术后骨折复位均满意.X线片显示骨缺损区修复良好,无骨溶解现象.结论:应用诱导骨基质植入治疗胫骨平台骨折疗效满意,该材料不仅具有一定的生物降解性能、良好的组织相溶性和稳定性,且具有诱导成骨作用。
Objective:To analyze the effect of implantation of BMG in the treatment of tibial plateau fracture. Methods:From April 1998 to August 2004,35 patients (male 22, female 13) with tibial plateau fracture were treated with implantation of BMG and fixation with L-trapaezoid compression plate (L-TCP) and lag screw. The age was 22 - 71 years with an average of 44. 3 years. Anterior and lateral X-ray and CT scan was done before operation. Operative program was defined according to type of fracture and condition of soft tissue. Results:All the patients were followed up from 6 to 48 months with an average of (28.6±8.2 ) months. According to criteria of M erchant:19 cases were excellent, 10 good,4 fair, 2 poor. Bone healing time was 8 - 15 weeks with an average of ( 11.3 ±1.6) weeks. Reduction was satisfactory in all patients. There was no X-ray showed plerosis of bony defect completely and osteolysis. Conclusion: Implantation of BMG in the treatment of tibial plateau fracture is effective. The material has good stability can induce osteogenesis.
