Virchow-Robin space (VRS), also called perivascular space, is a normal morphological structure in the central nervous system, and of some physiological and immunological implications. On MRI the VRS appears as well-defined punctate or linear foci with a smooth margin, with signal intensity similar to that of CSF on T1WI, T2WI and FLAIR sequences, conforming to the path of penetrating arteries, not showing contrast enhancement or mass effect. The presence of the enlarged or dilated VRS has been associated with hypertension, diabetes, cerebral arteriosclerosis, cerebral white matter diseases, hydrocephalus, multiple sclerosis, myotonic dystrophy, cryptococcal infection in central nervous system, and children with mental retardation.
