摘要
目的应用组织多普勒成像(TDI)技术评价获得性免疫缺陷综合征(AIDS)患者左心功能变化情况。方法入选105例血清反应阳性的人类免疫缺陷病毒(HIV)感染患者,根据病程分为HIV感染者组50例和AIDS患者组55例,另选取50例性别、年龄相匹配的健康志愿者作为对照组。于收缩期、舒张早期及舒张晚期测量3组二尖瓣环各位点在左室长轴上的峰值运动速度(Vs、Ve和Va),并作对比分析。结果与对照组相比,HIV感染者和AIDS患者二尖瓣环各位点Vs、Ve、Va均明显减低(P<0.01);与HIV感染者相比,AIDS患者二尖瓣环各位点Vs、Ve、Va均明显减低(P<0.01)。结论HIV感染及AIDS患者左室功能发生明显的损害。随着病程进展,其受损程度逐渐加重。利用TDI技术评估HIV感染及AIDS患者左心功能具有一定的临床价值。
Objective To assess left ventricular function in HIV-infected patients with tissue Doppler imaging. Methods One hundred and five HIV-infected patients were classified into two groups according to infection statuses of patients. One group consists of 50 asymptomatic HIV-seropositive patients, and the other group consists of 55 AIDS patients with complications. Fifty HIV-seronegative volunteers were recruited as normal controls. Peak velocities of six different sites selected in mitral annulus in the systole(Vs), the early diastole (Ve) and the late diastole(Va) were measured respectively in three groups and were analyzed comparatively. Results Compared with normal controls, HIV-seropositive patients and AIDS patients all showed significantly depressed Vs, Ve, Va( P 〈0.01 ) ; Compared to HIV-seropositive patients, AIDS patients showed significantly depressed Vs, Ve, Va( P 〈0.01). Conclusions Left ventricular dysfunction was significantly prevalent in HIV-infected patients and gradually deteriorated with advancing of course of disease. Tissue Doppler imaging is a useful technique on assessing left ventricular dysfunction in HIV-infected patients.
