人工髋关节置换术后假体无菌性松动的机械力学及生物学因素 被引量:7

摘要 目的:从机械力学因素及生物学因素两大方面分析假体松动的发病机制,并提出预防措施。资料来源:应用计算机检索EBSCO1999-01/2005-10相关人工髋关节置换术后假体无菌性松动的文献,检索词“Totalhiparthplasty;protheticappliancelooseness”,并限定文献语言种类为English。同时计算机检索CNKI1999-01/2005-10相关人工髋关节置换术后假体无菌性松动方面的文献,检索词为“人工髋关节置换术,假体松动”,并限定语言种类为中文。资料选择:对资料进行初审,选取包括人工髋关节置换术后假体无菌性松动方面的文献,开始查找全文。纳入标准:①人工髋关节置换术后假体无菌性松动。②临床研究文献。排除标准:①人工髋关节置换术后其他并发症。②术后感染导致的假体松动。③重复研究的文章。资料提炼:共收集到48篇关于人工髋关节置换术后假体无菌性松动的文献,纳入24篇符合标准的文献。资料综合:由于人工髋关节置换术后关节假体无菌性松动是影响人工关节长期稳定性、减少人工关节使用寿命的主要原因,故对其发生机制及预防进行综述性研究。人工髋关节置换术后假体无菌性松动的机制较为复杂,其主要原因是磨损颗粒,其次是力学因素,它们共同结果是造成骨吸收、骨溶解,最终导致人工髋关节术后假体部件的松动。其过程还与许多细胞因子的参与密切相关。针对引起关节假体无菌性松动的发病机制,可以从关节材料、抑制磨损颗粒移动、促进成骨及抑制骨吸收等方面来防治。结论:在临床治疗上,针对人工髋关节置换术后假体松动的各种因素,对其进行预防,可望减低发病率。 OBJECTIVE: To analyze the pathogenesis of prosthetic appliance from mechanical and biological factors, and provide preventive measures. DATA SOURCES: We researched EBSCO for prosthetic aseptic looseness after total hip arthroplasty related articles published from January 1999 to October 2005 on the computer, with the key words of "Total hip arthplasty; prosthetic appliance looseness"in English. We also searched CNKI for prosthetic aseptic looseness after total hip arthroplasty related articles published from January 1999 to October 2005 on the computer, with the key words of "Total hip arthplasty; prosthetic appliance looseness"in Chinese. STUDY SELECTION: The articles were selected primarily, including the articles of prosthetic aseptic looseness after total hip arthroplasty , and their full-text was looked for. Inclusive criteria: ①Prosthetie aseptic looseness after total hip arthroplasty. ② Articles of clinical study. Exclusive criteria: ① With other complications after total hip arthroplasty.② Prosthetic looseness caused by postoperative infection. ③ Repetitive articles. DATA EXTRACTION: Totally 48 articles related to prosthetic aseptic looseness after total hip arthroplasty were collected and 24 enrolled articles met the criteria. DATA SYNTHESIS: Because prosthetic aseptic looseness after total hip arthroplasty were the major factors influencing the long-term stability of artificial hip arthrosis and reducing the service life of artificial hip arthrosis, therefore, their mechanism and prevention were reviewed. The mechanism of prosthetic aseptic looseness after total hip arthroplasty was complex, and its main factor was worn particles, and the second was mechanical factors. Bone absorption and bone dissolution were the common results, which resulted the prosthetic aseptic looseness after total hip arthroplasty, The process was also closely related to the participation of many cytokines .As to the pathogenesis of prosthetic aseptic looseness, we could prevent it from arthrosis materials, inhibiting the movement of worn particles, promoting bone formation and inhibiting bone absorption. CONCLUSION: It is possible that we can decrease its morbidity through prevention based on the mechanism of aseptic looseness Of prosthetic components after the total hip arthroplasty in clinic.
