幽门螺杆菌感染者胃IL-4和IFN-γ表达的免疫组织化学研究 被引量:4

摘要 目的探讨幽门螺杆菌感染与未感染的十二指肠溃疡(DU)、胃溃疡(GU)、慢性胃炎(CG)患者的胃白介素-4(IL-4)和γ-干扰素(IFN-γ)的原位分布,观察其间的差异.方法选取接受胃镜检查并活检的门诊病人的胃粘膜标本97例进行H.pylori感染的评定,免疫组织化学染色观察IL-4和IFN-γ在胃的原位分布.结果IFN-γ、IL-4在Hp感染组与未感染组均有表达,IL-4主要分布于固有层的淋巴细胞和单核细胞以及少量的胃上皮细胞.IFN-γ分布于部分上皮细胞、固有层单核细胞和淋巴细胞.Hp感染者中GU、CG患者IFN-γ的染色阳性率高于GU,差异有统计学意义(P=0.019);Hp感染者IL-4染色阳性率在DU、GU、CG间无差别(P>0.05).结论无论是否存在Hp感染,胃粘膜都有IL-4和IFN-γ表达,染色阳性率与是否感染无关;IFN-γ的表达在GU、CG的表达较DU明显,提示其与GU、CG的关系更为密切. Objective To explore the expression of the proinflammatory interleukin -4 and the immunoregulatory gamma interferon ( IFN - r) in stomach of antral biopsy specimens from helicobacter pylori ( H. pylori ) -infected and uninfected duodenal ulcer (DU) patients, gastric ulcer (GU) and with chronic gastritis only. Methods The immunohistochemical technique was used to determine the distribution and differences of these cytokines in situ from H. pylori - infected specimens and uninfected controls. Results The expression of IL-4 and IFN -γ were observed in both H. pylori -infected subjects and uninfected controls. IL- 4 was seen to be mainly located in lymphocytes, monocytes and glandular epithelia cells. Comparatively, IFN -γ was located in lymphocytes, monocytes in lamina propria as well as glandular epithelial cells. In addition, the positive rate for IFN - r in GU and CG is higher than that seen in DU ( P 〈0. 05). Conclusions The expression of IFN -r and IL -4 is not directly related to H. pylori infection of the patients of the three groups. However, after H. pylori infection, the expression of IFN - γ in CG ands GU is more than that of DU, which suggested the possible relationship between the level of IFN - γ and H. pylori - associated diseases.
