摘要 目的 探讨冠状动脉造影术中造影剂对肾功能的影响。方法 ①按患者所用造影剂的量分成50~100ml组、100-200ml组和〉200ml组。②分别测定术前、术后24h、术后48h、术后10d的血尿素氮(BLJN)、血肌苷(Cr)、尿微量白蛋白(MAU)、尿β2-微球蛋白(β2 MG)的值并进行比较。结果 造影后24h、48h尿β2 MG与造影前及造影后10d比较有显著性差异(P〈0.05),其余指标各时段之间比较无显著性差异(P〉0.05)。造影后24h、48h造影剂〉200ml组与50~100ml组及100-200ml组之间Cr、MAU及陉MG的变化有显著性差异(P〈0.05)。其余各组之间比较无显著性差异(P〉0.05)。结论 造影剂对肾功能有一定的损害,并且随造影剂剂量的增加其损害加重。 Objective To investigate the relation between contrast medium and kidney function in coronary angiography. Methods ① According to the dosage of contrast medium, patients were divided into three groups: 50- 100ml, 100-200ml and above 200 ml. ② Blood BUN,Blood Cr, albuminuria and urine β2 MG levels of all patients were measured before coronary angiography and after coronary angiography 24h,48h,10d. Results The levels of urine β2 MG after coronary angiography 24h and 48h were higher than before coronary angiography and after coronary angiography 10d(P 〈 0.05), the other groups were no significantly different in statistics(P〉0.05). The Blood Cr,Albuminuria and urine β2 MG levels after coronary angiogaphy 24h and 48h in the group above 200 ml were higher than in the group 50- 100ml and 100-200ml(P 〈 0.05), the other groups were no significantly different in statistics(P 〉0.05). Conclusion Contrast medium injures kidney in coronary angiography and the deterioration of kidney function is related with the dose of contrast medium.
