非小细胞肺癌组织中bcl-2蛋白的表达与预后 被引量:2

摘要 目的研究抗凋亡基因bcl-2在非小细胞肺癌组织中的表达、相互关系及对预后的影响和意义。方法应用SP法检测94例非小细胞肺癌的石蜡包埋标本中bcl-2蛋白的表达,同时取10例正常肺组织作对照。结果在非小细胞肺癌组织中,bcl-2蛋白在鳞癌中的阳性率为63·64%(35/55),腺癌中为41·02%(16/39),两者差异无统计学意义。χ2=3·83,P>0·05;bcl-2蛋白在中分化的鳞癌和腺癌中阳性率为61·11%(22/36),在低分化的鳞癌和腺癌中阳性率为64·86%(24/37),与高分化的鳞癌和腺癌(23·81%,5/21)分别相比差异有统计学意义,χ2=5·98,P<0·05和χ2=7·46,P<0·01;bcl-2阳性表达患者3年生存率为27·45%(14/51),而阴性表达患者3年生存率为51·16%(22/43),两者差异有统计学意义,χ2=4·59,P<0·05。在正常肺组织标本中,表达为阴性。结论bcl-2蛋白高表达患者临床预后差,故bcl-2蛋白的表达可作为判断预后指标之一。 OBJECTIVE: To investigate the expression of bcl-2 in primary non-small cell lung carcinoma (NSCLC) and to investigate the correlations between them as well as the effects on the prognosis of NSCLC. METHODS: bcl-2 protein expressions were detected in the specimens of 94 NSCLC and 10 normal lung tissues by the immunohistochemistry technique. RESULTS: The positive rate of bcl-2 protein in squamous cell carcinomas of lung was 63.64% (35/55), and the positive rate in adenocarcinomas was 41.02% (16/39), X^2 = 3.83, P 〈 0.05. The positive rate of bcl-2 protein in squamous cell carcinomas and adenocarcinomas of lung with moderately differentiate was 61.11% (22/36), in carcinomas with poory differentiation was 64.86% (24/37), correlated with carcinomas with high differentiation (23.81%, 5/21) separately, X^2 = 5.98, P 〈 0.05 and X^2 = 7.46, P 〈 0.01. The three years' survival rate was 27.45% (14/51) in the cases with bcl-2 protein positive expressions, while it was 51.16% (22/43) in the cases with bcl-2 protein negative expressions, X^2 = 4.59, P 〈 0.05. Negative results were observed in all normal lung tissues. CONCLUSION: There are poor prognoses of patients who have high expressions of bcl-2, so bcl-2 protein expression is a useful indicator predicting the prognosis.
