腹腔镜胆囊切除术中不同气腹压力对肾功能影响的对比观察 被引量:24

摘要 目的:研究腹腔镜胆囊切除术(LC)中不同气腹压力对肾功能的影响。方法:随机从术前肾功能正常及异常均欲行LC手术的患者中抽取两组,分别定为A组和B组,每组再随机设定在低气腹压(8-12mmHg)或高气腹压(13~15mmHg)状态下手术。结果:术前肾功能正常及异常者在不同气腹压力下(8-15mmHg)行LC,低压时对肾脏功能影响不大,高压时部分肾功能正常的患者的CO2-CP值会有所下降,肾功能异常者的有较明显的影响。结论:对肾功能异常欲行LC者,术中应选择低气腹压力。 Objective :To study whether different pressure has difference on kidney function or not in laparoscopic cholecystectomy(LC). Methods:Before operation, patients who were desired to LC surgery were chosen randomly from the patients with normal and abnormal kidney function,and then they were assigned randomly into A and B groups,each group was randomly received LC operation alternatively on the condition of low pressure pneumoperitoneum (8-12mm Hg) or high pneumoperitoneum (13-15mm Hg). Resuits : As for the patients with normal kidney function before surgery, low pressure made little effect on kidney function, and the value of CO2-CP decreased in a small part of patients as the pressure was high. As for the patients with abnormal kidney function, the effect was not significant on the condition of low pneumoperetoneum, however it was significant on the condition of high pneumoperitoneum. Conclusions :As for the patients with abnormal kidney function who are desired to LC, it is better to choose the condition of low pneumoperitoneum.
