摘要
目的研究了经颅多普勒超声与计算机断层扫描血管成像在诊断无症状性颈内动脉狭窄或闭塞中的作用。方法对19例经全脑数字减影血管造影确诊的颈内动脉严重狭窄或闭塞患者进行经颅多普勒及计算机断层扫描血管成像检查,与数字减影血管造影对比分析。结果经颅多普勒超声与计算机断层扫描亦管成像完全符合14例,2例前交通支、3例后交通支评价不足。患侧大脑中动脉的最高流速均低于健侧,分别为58±21.3和86±28.6 cm/s(t=10.51,P<0.01),双侧颅内血管搏动指数不对称,患侧0.53±0.153,健侧1.0±O.182(t=13.705,P<0.01)。计算机断层扫描血管成像与数字减影血管造影完全相符14例,不一致5例。结论经颅多普勒超声与计算机断层扫描血管成像对颈内动脉狭窄或闭塞具有较大诊断价值,但有局限性,两者结合使用,在诊断颈内动脉狭窄或闭塞中具有互补作用,可以提高评价血管病变程度的准确性。
Objective Diagonsis no symptom narrow and blocking in neck artery had been stadied by means of the transcranial cerebral doppler(TCD) and CT blood vessel angiography(CTA). Methods A comparative detection for 19 cases patients. Whom were comfired to be sreiovs artery narrow and blocking by the digital substractive blood vessel angiography(DSA) had been undertaken. Results The result indicated that the insufficient estimation was only for 2 cases ACOA and 3 cases PCOA, 14 cases were well consistent with the results of DSA. The largest current velocity of MCA in ill side is 58±21.3 cm/s, apparently lower than 86±28.6 cm/s for healthy sid(t = 10.51, P 〈 0.01). And the PI values in both side of cranium are asymmetric, which is 0.53±0.153 for ill side and 1.0±0.182 for healthy side(t = 13.705, P 〈 0.01). Conclusion These results have proved that the synergetic application of TCD with CTA can improve the accuracy for evaluating the degree of pathological changes in blood vessel, and the method has a high value to diagnose the narrow and blocking in neck artery.
