摘要
目的:探索开胸高位结扎前降支建立急性大面积心肌梗塞模型的适宜方法。方法:中国小型猪18头全麻下开胸暴露心脏,预阻断冠状动脉前降支(第一对角支下)距左主干1.5 cm处3次,5 min/次,后予结扎,应用心电图动态监测,超声心动图,病理组织切片及99mTc-MIBI心肌断层显像等确定模型建立情况。结果:18头中国小型猪中有8头存活4周,1头麻醉意外死亡,1头存活5 d,1头存活3 d,其余在术中室颤死亡。结扎后心电图呈动态变化,24 h后血清心肌酶谱(谷草转氨酶、乳酸脱氢酶、磷酸肌酸激酶)较术前明显增高,差异有统计学意义(P<0.01)。4周后超声心动图与术前比较EF、LVDd、LVDs、PWTI、VST有显著性差别(P<0.05),99mTc-MIBI心肌断层显像示均有不同程度心肌缺血,DF为29.57±2.88。结论:该法操作简单,重复性好,形成梗塞面积大,术后可长期饲养,为临床冠心病的实验研究提供了一个有价值的动物模型。
Objective: To explore an appropriate method to establish an animal model of acute extensive area myocardial infarction with ligation of the left anterior descending(LAD) coronary artery in high position. Methods: Eighteen normal Chinese miniswine underwent thoracotmy under general anesthesia. The LAD was obstructed in advance for 3 times and then was ligated after the opening of the first diagonal branch. Acute myocardial infarction created by this technique was confirmed by electrocardiogram,echocardiogram,^99mTc-MIBI myocardial SPECT and pathological examination. Results:There were 8 pigs surviving for 4 weeks,one for 5 days,one for 3 days. One died for anesthesia accident and the others died for ventricular fibrillation during operation. EKG displayed ST elevated continuously and pathologic Q wave after ligation. Serum myocardial zymogram including AST,CPK and LDH increased 24 hours later and had significant difference(P〈 0. 01) compared with that before operation. Echocardiogram showed obvious decrease of cardiac function and EF,LVDd, LVDs, PWT, IVST were significantly different with those before opera- tion(P〈0.05). ^99mTc-MIBI myocardial SPECT showed myocardial ischemia and infarction in varying degrees. DF was 29. 57±2.88. Conclusion:The method is simple and well repeated. The formation of myocardial infarction is reliable and animals can be maintained for a long time after operation. It provides a valuable animal model for the experimental study of coronary artery disease.
