摘要
目的:比较3种化学方法处理异体小血管移植后血管通畅状态以及病理变化。方法:实验于2005-03/09在中山大学医学院动物实验中心完成。①采用随机数字表法将64只新西兰白兔随机分成4组,自体组,甘油组,乙醇组和戊二醛组,每组16只。②自体组以自体股动脉移植作为对照;甘油组,乙醇组和戊二醛组,分别将980g/L甘油、体积分数为0.95的乙醇、5g/L戊二醛保存的异体兔股动脉,移植桥接于兔股动脉。③各组兔分别于术后7,14,28,56d观察血管通畅情况,同时取移植体标本观察其病理变化。结果:①各组兔股动脉通畅率:甘油组和自体血管移植8周后通畅率均为100%;乙醇组8周后通畅率为69%,戊二醛组通畅率为6%;甘油通畅率明显高于乙醇组和戊二醛组(P<0.01)。②各组兔移植前血管组织学变化:甘油浸泡后的血管复苏后外观颜色、质地与正常血管相同,血管弹性良好。乙醇浸泡后的血管其外观颜色、质地与正常血管相同,但血管弹性稍差。戊二醛组血管颜色略变黄,血管弹性欠佳。③异体血管移植后组织学检查结果:甘油组术后2周开始,血管内壁内皮细胞开始增生,至8周时明显。弹力纤维8周时仍可见排列尚整齐。乙醇组术后移植血管内壁内皮细胞变化基本同甘油组。中层平滑肌细胞以及弹力纤维移植后2周开始出现变性或结构紊乱。戊二醛组从2周开始,管腔封闭,弹力纤维结构紊乱。4~8周管腔内充满较多的成纤维细胞。结论:3种化学方法中,甘油处理的异体小动脉移植在8周内与自体血管移植通畅率无明显差别,在8周内可保证通畅。
AIM: To compare the pateney and histology of small vascular allografts preserved by three different chemical materials and its pathological changes. METHODS: The experiment was conducted in the Experimental Center of Animal, Medical College of Sun-Yet sen University between March and September 2005. (1)64 rabbits were randomly divided into four groups, auto-transplantation group, glycerin group, alcohol group and glutaric dialdehyde group (GA group) with 16 rabbits in each group.(2)Rabbits in the autoransplantation group were transplanted by the femoral artery of themselves and taken as control. Rabbits in the alcohol and GA group respectively received 980 g/L glycerin, 0.95 of alcohol and 5 g/L of GA to preserve rabbit femoral artery of allograft, and was transplanted to the femoral artery of rabbits. (3)The patency and histology of vessels in rabbits of all groups were observed respectively at 7, 14, 28 and 56days after operation, and specimens of transplants were obtained at the same time to observe its pathological changes. RESULTS: (1)The patency fate of femoral artery in rabbits of all groups: that in the auto-transplantation group and glycerin group were 100% at 8 weeks later, while that in the alcohol group was 69%, and that in the GA group was 6%, and it was obviously higher than the alcohol group and GA group (P 〈 0.01).(2)Histological changes of vessels before transplantation in rabbits of all groups: The external color and texture of vessels immersed into glycerin were the same as normal vessels after revitalization, and the vascular elasticity was good. The external color and texture of vessels immersed into alcohol were the same as normal vessels, while the vascular elasticity was a little bad. Vessels in the GA group slightly turned to be yellow, and the elasticity was much worse.(3)Histologieal examination after vascular transplantation of allograft: from the 2^nd week after operation, the endothelial cells (ECs) in the inner wall of vessels in the .glycerin group began to proliferate, which was obvious at the 8^th week. The elastic fibers were still in order at the 8^th week. The changes of ECs after transplantation in the alcohol group were basically the same as glycerin group. The smooth muscle cells (SMCs) of intercellular layer and the elastic fibers began to degernerated and were in disorder from the 2nd week after transplantation. In the GA group, lumina sealed and elastic fibers were in disorder from the 2^nd week. In 4-8 weeks, there were much fibroblasts in the lumina. CONCLUSION; Among three different chemical materials, the pateney rate of small artery transplantation of allografts is not significantly different to that of auto-transplantation within 8 weeks, and the pateney can be maintained within 8 weeks.
