摘要
目的探讨乳腺癌变过程细胞凋亡和增殖的变化。方法用末端转移酶介导的脱氧核苷酸末端标记法(TUNEL法)和免疫组化法,检测乳腺癌组31例、乳腺癌前病变组43例(其中乳腺不典型增生21例,乳腺单纯性增生22例),正常乳腺组11例的凋亡指数、增殖细胞核抗原指数和bcl-2蛋白表达强度的变化。结果乳腺癌组凋亡指数和增殖细胞核抗原指数(5.36±0.61和40.84±9.99)低于乳腺癌前病变组(9.97±1.50和48.68±7.78),但高于正常乳腺组的凋亡指数和增殖细胞核抗原指数(2.66±0.49和22.87±6.98),差异有统计学意义(P<0.01);bcl-2蛋白表达分值为2.81±0.88、1.38±0.76、0.50±0.67,以乳腺癌组最高(P<0.01)。对乳腺癌组凋亡指数与PCNA、凋亡指数与bcl-2分值和PCNA与bcl-2分值行等级相关分析,相关系数rs分别为:0.832、-0.608和-0.475。结论乳腺组织细胞凋亡与增殖的异常表达预示着恶性转变可能发生。
Objectives To explore the change of apoptosis and cell proliferation in breast tissue carcinogenesis. Method Using TUNEL and immunohistochemical method , the differences of apoptosis index(AI) and bcl-2 and PCNA index(PI) were examined in 31 case with breast cancer and 43 with its precancerous lesion and 11 with normal tissues. Result The AI and expression of PCNA in breast cancer group were lower than the precancerous lesion(compared 5.36 ± 0.61 to 9.97 ± 1.50, and compared 40. 84±9.99 to 48.68±7.78, P〈0.01) and were higher than the normal group (compare 9.97± 1.50 to 2. 66±0.49, P〈0.01). expression of bcl-2 protein in breast cancer group were the highest than the precancerous lesion and the normal group (compared 2.81±0.88 to 1.38±0.76, and 0.50±0.67, P〈0. 01). There is positive relationship between AI and PI(rs=0. 832, P〈 0.01). There is negative relationships between AI and expression of bcl-2 protein, and between PI and expression of bcl-2 in breast cancer group by degree relative analysis(rs=-0.608 and-0. 475, P〈 0. 01).Conclusion The abnormal expression of apoptosis and cell proliferation in breast tissue indicated that it maybe occurring malignant.
