摘要
目的了解髋关节置换术中电刀与血液回收机联用对血一氧化碳(CO)水平的影响。方法对49名髋关节置换术中联用电刀与血液回收机的患者,多时点检测其动脉血及回收血中碳氧血红蛋白(CHbO2)含量。结果输入回收血的35名患者,经洗涤的回收血中CHbO2含量显著高于动脉血术前和术毕时浓度(P<0.01),术后6h CHbO2含量亦较术前和术毕时升高(P<0.05),7名原有心脏病史的患者中有2名术后出现心脏损害表现;未输入回收血的14名患者,术后6h CHbO2含量与术前和术毕时比较无显著性差异(P>0.05)。结论髋关节置换术中联用电刀与血液回收机并输注回收血可导致患者体内CHbO2含量升高。建议对原有心血管疾病的患者,应监测回收血CHbO2含量,并谨慎回输回收血。
Objective To evaluate the blood levels of carbon monoxide (CO) during hip arthroplasty with simultaneous use of electrocautery and a cell saver system. Methods Carboxyhemoglobin (CHbO2) levels were measured in 49 patients who underwent hip arthroplasty with the use of cell saver and electrocautery. The CHbO2 levels of salvaged blood collected by the cell saver system were compared to the systemic CHbO2 levels. Results Thirty-five patients received the salvaged blood. The CHbO2 level in salvaged blood was higher than those measured preoperatively and postoperatively ( P〈0.01). A significant increase in the systemic CHbO2 concentration was also observed 6 hours after the operation ( P〈0.05). Two patients with pre-existing heart disease demonstrated cardiovascular compromise postoperatively. Fourteen patients who did not receive any salvaged blood, demonstrating no difference among all the levels of CHbO2 detected ( P〉0.05). Conclusion The simultaneous use of electrocautery and a cell saver system in hip arthroplasty may lead to a significant increase of systemic CHbO2 concentration. CHbO2 level in salvaged blood should be measured and the transfusion of salvaged blood to patients with pre-existing cardiovascular disease should be cautious.
