摘要
目的了解急诊死亡的病因构成和救治状况，为制定预防措施提供依据。方法对我院2005年全年死亡病例进行回顾性分析，对以上患者的性别、年龄、发病情况、病因、抢救地点、抢救时间、抢救措施等资料进行整理。结果平均死亡年龄为68．4岁，以70—80岁年龄段最多。死亡原因依次为脑血管病、呼吸系统疾病、猝死、心脏病、恶性肿瘤等，慢性病及心脑血管病占70％；院内猝死占7．8％；诊断不清者占15．7％。与家属提出不复苏指令的相关因素为年龄增大、救治时间延长．猝死是家属不放弃抢救的重要因素。结论慢性病及心脑血管疾病占急诊死亡病例的绝大部分；应采取措施积极预防院内猝死的发生；应重视急诊疑难病。
Objective To analysis the causes and the treatment procedures of the died cases in the emergency room to find more evidences for making preventive measure of sudden death. Methods To conclude and analysis the data of 115 died cases in our emergency room in 2005 retrospectively, such as sex, age, incidents, causes, the procedures and measures of treatment. Results ① The average age of the died cases was 68.4 years old, most of them died from 70 to 80; The causes of death were cerebrovascular disease, disease of respiratory system, Cardiac disease, malignant tumor by turns; 70% of them died from deterioration of the chronic diseases and cerebrovascular incidents; 26.9% of the patients below 60 years old still remained unclear about cause of death. ② 7.8% of the total died cases were sudden death cases happened outside the emergency room inside the hospital. ③ Sudden death was the important reason that the patients relatives asked to persist resuscitation, and the correlation factors that the relatives asked to give up resuscitation were patient＇s age and the long time of rescue. Conclusions ① Most of the died cases in the emergency room are died from deterioration of chronic disease and cardio-cerebrovascular incident. ② Sudden death outside the emergency room in the hospital should not be ignored, but actively prevented. ③How to make a definite diagnosis in time is still remained one of the important issue the emergency doctors have to solve.
