新生儿重症监护病房血培养结果分析

摘要 目的研究新生儿重症监护病房(NICU)病原菌及耐药情况。方法无菌抽取疑似败血症患儿血液进行培养、分离、鉴定及药敏实验。结果①860份普通血培养分离细菌16种185株,总阳性率21.5%,其中G+球菌155株,以凝固酶阴性葡萄球菌为主,金黄色葡萄球菌次主,G-杆菌10种24株;②530份L型细菌培养共分离出细菌6种95株,总阳性率为17.9%;其中G+球菌86株,以金黄色葡萄球菌为主,凝固酶阴性葡萄球菌次之;③G+球菌对青霉素、苯唑西林、克林霉素及阿奇霉素有较高的耐药性,G-杆菌对头孢噻肟、头孢唑啉、哌拉西林、复方新诺明有较高的耐药性。结论①凝固酶阴性葡萄球菌、金黄色葡萄球菌、肠球菌是NICU中新生儿败血症或菌血症的主要病原菌;②所有细菌对常用抗生素产生较高的耐药性。 Objective To study sive care unit(NICU). Methods the distribution of pathogens and their drug - resistance in neonatal intenBlood samples from patients who suspected with septicemia were cultured and isolated. All of the isolates were tested by Kirby - Bauer susceptibility and their resistance were analyzed according to NCCLS standard. Results ①A total of 16 species and 185 pathogenic strains were isolated from 860 specimens, and the positive rate was 21.5% , Among them, 155 strains were gram -positive coccus and 30 strains gram -negative bacilli. Among gram -positive coccus, the most dominant organism was coagulase negative staphylococcus and the next was staphylococcus aureus②A total of 6 species and 95 pathogenic L - strains were isolated from 530 L - specimens, and the positive rate was 17.9%. Among them 86 L - strains were gram positive coccus and 9 L - strain gram - negative bacilli. Amorng gram - positive coccus, the most dominant organism was staphylococcus and the next was coagulase negative staphylococcus. ③The resistance rates of gram - positive coccus to penicillin G,oxacillin, clindamycim and azitromycin were rather higher. The resistance rates of gram - negative bacilli to cefotaxime, cefazolin, piperacillin and sulfamethoxazole/trimethoprim were rather higher. Conclusion ①Coagulase negative staphylococcus, staphylococcus aureus and enterococcus are the primary pathogens for neonatal septicemia/bacteriemia in NICU. ②The rate of drug resistance in different bacteria was rather higher.
