糖代谢异常人群胰岛分泌功能及餐后血糖影响因素研究

摘要 目的研究糖代谢异常人群胰岛分泌功能的差异以及空腹血糖(FPG)和空腹胰岛素(FIns)对餐后2 h血糖(2 h PG)的影响。方法选取在同济医院内分泌实验室做口服糖耐量试验(OGTT)和胰岛素释放试验(IRT)的病人525例。用HOMA胰岛素敏感指数(ISI)评价胰岛素抵抗程度、胰岛素分泌指数(IR I)评价胰岛β细胞的分泌功能。广义线性回归模型分析自变量FPG和FIns对因变量2 h PG的影响,协方差分析比较不同人群胰岛分泌功能。结果在DM组中FPG和FIns的标准偏回归系数分别为0.4998,0.5106,IGT+IFG组中FPG和FIns的标准偏回归系数分别为0.6452,0.3749。协方差分析显示,DM组人群IR I与IGT+IFG组无显著性差异(F=2.14,P=0.1384)。结论DM人群中FPG和FIns对2 hPG影响贡献度大小相近;在IGT+IFG组中,FPG对2 hPG的影响贡献度大于FIns。IGT+IFG组人群胰岛分泌功能较IFG和IGT组人群下降。早期发现人群中潜在IGT+IFG人群,并进行早期生活方式以及胰岛素增敏剂的干预治疗以改善胰岛素抵抗和控制空腹血糖,对于餐后血糖的达标具有重要意义。 [ Abstract] Objective To investigate the difference of secretion function of insulin among people with glucose metabolic dysfunction and the relationship between FPG and 2hPG among differ- ent groups with glucose metabolism dysfunction. Methods 525 cases in Tongji hospital were re- cruited to undergo OGlE and IRT. Insulin sensitivity index (ISI) is applied to represent the level of insulin resistance and insulin release index (IRI) is applied to evaluate the secretion function of 13 cell. Results In DM group the standard partial regression coefficient of FPG and Fins is 0. 6542,0. 3749 and in the IGT + IFG group is 0. 6452,0. 3749 respectively. Analysis of covariance indicated there is no significance in IRI between IGT and IFG group. Conclusion In DM group the contribution of FPG and Fins to 2hPG is similar. Contribution to 2hPG of FPG is higher than Fins. Early detection of latent IGT/IFG among people, interference of styles of life and treatment with insulin sensitizer to control fasting blood glucose is of great significance.
