摘要
目的与左心导管法对比探讨定量组织速度成像(QTVI)超声心动技术评价高血压患者左心室收缩、舒张功能的价值。方法21例高血压患者在超声心动图检查结束后24小时内行左心导管检查。QTVI测量左室壁(侧壁和后间隔、前壁和下壁、前间隔和后壁)二尖瓣环6个位点处收缩期峰值速度(Sm)、舒张早期峰值速度(Em)、舒张晚期峰值速度(Am)。左心导管检查测定左心室收缩期压力上升最大速率(dp/dt)m ax、左心室舒张期压力下降最大速率[(dp/dt)m in]、计算等容舒张期左心室压力下降时间常数T值。结果高血压患者左心室壁二尖瓣环6个位点处的平均峰值S即MSm与(dp/dt)m ax和左室射血分数(LVEF)均有显著正相关性,分别为(r=0.77,P<0.01;r=0.61,P<0.05)。左心室壁二尖瓣环6个位点处的平均峰值Em即MEm与(dp/dt)m in和T呈显著的负相关性,分别为(r=-0.68和r=-0.76,P<0.01);E/A与(dp/dt)m in和T以及Em无统计学意义的相关性(P>0.05)。结论QTVI所测MSm及MEm可为临床提供一种新的无创、简便、准确评价左心室收缩和舒张功能指标。
Objective To investigate the quantitative correlation of routine dynamic indexes between quantitative tissue velocity imaging(QTVI) and left cardiac catheterization in essential hypertension. Methods Within 24 hours after echocardiographic imaging, left cardiac catheterization was performed in 21 patients with essential hypertension. The peak velocities of six left ventricular wall sites at mitral annuluses during systole, early and late diastole(Sm, Em, Am) were measured, and the peak positive and negative rate of pressure development(dp/dt) were measured by cardiac catheterization. Results Mean Sm had a good correlation with peak positive dp/dt and( r = 0.77, P〈0.01), as well as with left ventricular ejection fraction( r =0.61, P〈0.05). Left ventricular mean Em correlated significantly with negative dp/dt and T( r = -0.68 and r =-0.76 P〈0.01 respectively) ,while E/A had no significant correlation with negative dp/dt and T and mean Em( P〈0.05). Conclusion QTVI is a new, quantitative and noninvasive method to assess left ventricular systolic and diastolic function in patients with essential hypertension.
