熄风化痰活血法治疗痰湿阻络型椎动脉型颈椎病临床观察 被引量:9

摘要 根据痰湿阻络型椎动脉型颈椎病发病机理及临床表现,在熄风化痰剂半夏白术天麻汤的基础上加入活血化瘀中药,治疗痰湿阻络型椎动脉型颈椎病(治疗组)50例,并以采用半夏白术天麻汤治疗的40例为对照(对照组)。治疗3周后,根据《中医病证诊断疗效标准》评定,结果治疗组治愈率为80%,总有效率为92%;对照组治愈率为52%,总有效率为85%。两组比较,治疗组较对照组治愈率明显提高(P<0.01)。TCD检测显示,两组治疗后BA、LVA、RVA的血流速度均较治疗前明显提高,且治疗组较对照组更为显著(P<0.05)。表明痰湿阻络型椎动脉型颈椎病的病机特点应痰瘀阻络,风痰上扰。治宜熄风化痰,活血化瘀。 According to the pathogenic mechanism and clinical manifestations of cervical spondylosis of vertebral-arterial type (CSA) due to channel obstruction by phlegm-dampness (COPD),50 cases of CSA due to COPD (the treated group) were treated by Decoction of Pinellia,Bighead Atractylodes and Gastrodia (DPBAG) plus blood-circulation-activating and blood-stasis-eliminating TCD,and 40 cases of this disease treated by DPBAG alone were taken as the control group. After the treatment of three weeks,the results were evaluated according 《The Diagnostic and Therapeutic-effect Standards of TCM Diseases》and showed that the cured and total effective rates of the treated and the control were 80 % and 92 % versus 52 % and 85 %,respectively,the treated group having an obvious advantage over the control in the cured rate (P < 0.01). The TCD test showed that the blood flow rates of BA,LVA and RVA of the two groups were significantly improved in comparison the pre-treatment ones,the treated having an obvious advantage over the control (P < 0.05),suggesting that CSA due to COPD is characterized by phlegm-dampness channel obstruction and wind-phlegm upward intervention and should be treated by the methods of calming the wind,resolving the phlegm,activating the blood circulation and eliminating the blood stasis.
