胃癌患者骨髓微转移细胞的形态及其临床意义 被引量:2

摘要 目的探讨应用免疫磁珠富集技术研究胃癌骨髓微转移细胞的形态学分类及其临床意义。方法选择20例手术的胃癌患者,提取骨髓有核细胞后,以结合有抗人上皮抗原(hum an ep ithe lia l an tigen,HEA)抗体的M ACS微型免疫磁珠进行标记,以M S+/RS+型阳性分选柱进行活性肿瘤细胞富集两次。以标记异硫氰酸荧光素(fluoresce in isoth iocyanate,F ITC)的抗细胞角蛋白(cytokeratin,CK)抗体CK-F ITC、二氨基苯茚酮以及溴化乙锭(eth id ium brom ide,EB)染色后,观察阳性细胞形态。结果全组共有9例患者骨髓中检测到344个肿瘤细胞。其中,中等细胞型192个(55.8%),大细胞型51个(14.8%),干细胞样细胞23个(6.7%),M期细胞4个(1.2%),有核细胞碎片42个(12.2%),无核细胞碎片32个(9.3%)。骨髓微转移细胞数与肿瘤TNM分期有关(P=0.038),而与性别(P=0.659)、年龄(P=0.500)、组织学分级(P=0.351)和淋巴结转移情况(P=0.060)无关。结论胃癌骨髓微转移细胞形态多样,预示预后较差。
