摘要
目的探讨糖皮质激素受体α(GRα)在脑膜瘤的表达及在脑膜瘤发生发展中的意义。方法应用流式细胞术(FCM)检测58例脑膜瘤的增殖指数(PI)、S期细胞比率(SPF)、DNA倍性以评价肿瘤细胞的增殖活性,应用RT-PCR法检测脑膜瘤与7例正常硬脑膜GRα的表达,比较脑膜瘤与正常硬脑膜GRα表达的差异,并分析GRα表达与脑膜瘤病理级别、PI、SPF、DNA倍性的关系。结果58例脑膜瘤的PI为9·32%±4·41%,SPF为2·79%±2·43%。其中二倍体51例,异倍体7例,异倍体率为12·1%。GRα“-”9例,“+”13例,“”15例,“”9例,“”12例,总的阳性率为84·5%。而7例正常硬脑膜均为GRα阴性表达,脑膜瘤的GRα阳性率显著高于正常硬脑膜。GRα阴性、弱阳性、强阳性这三组脑膜瘤的PI、SPF均无显著差异,GRα阳性与阴性脑膜瘤的异倍体率也未见差异。结论脑膜瘤组织是糖皮质激素的靶组织,GRα在脑膜瘤的发生中具有一定的意义;在诸多影响因素之中,GRα不是调节脑膜瘤增殖的主要因素,它的表达对脑膜瘤细胞的增殖活性无显著影响。
Objective To explore the expression of glucocorticoid receptorα (GRα) and its significance in the occurrence and progress of meningioma. Methods The proliferative index (PI), S-phase fraction (SPF) and DNA ploidy of 58 meningioma specimens were detected by flow cytometry to evaluate the proliferation of tumor cells. The expression of GRα in all meningioma and 7 normal dura specimens were studied by reverse transcription-polymerase chain reaction to compare the difference in GRαbetween meningioma and normal dura. The relation between GRαand histological grade, PI, SPF and DNA ploidy were also analyzed. Results The mean PI of 58 meningioma cases was 9.32%±4.41%, and the mean SPF was 2.79%±2.43%. DNA of 51 cases was diploid and that of 7 aneuploid, the aneuploid rate being 12.1%. 9 of 58 cases were GRα-negative and the others were GRα-positive, the positive rate being 84.5%^. In GRα- positive cases 13 were "+" 15 "++", 9 "卅" and 12 "卌". GRα of 7 normal dura was negative. The GRα- positive rate of meningioma was significantly greater than that of normal dura. There were no significant differences in PI and SPF among GRα-negative, GRα-weak positive and GRα-strong positive meningiomas. The difference in aneuploid rate between GRα-positive and GRα-negative meningiomas was also not significant. Conclusion Meningioma is the target tissue of glucocorticoid. GRα is of certain significance in the occurrence of meningioma. However, GRα is not the main factor to regulate the proliferation of meningioma. Its expression has no obvious effect on the proliferative activity of meningioma.
