摘要
目的分析青少年屈光不正性弱视使用硬性透气角膜接触镜(rigid gas permeable,RGP)在三年中的视力矫正效果以及在屈光度、图形视觉诱发电位(pattern-visual evoked potential,P-VEP)方面的变化及其机制。方法对23例年龄≥12周岁屈光参差性弱视患者[男10例15眼,女13例17眼,平均年龄为(15.26±2.01)岁]在实验配戴RGP始末分别测定屈光度、矫正视力和P-VEP;所有RGP实验对象配戴镜片周期〉50周,且与同期20例青少年屈光不正性弱视框架镜验配者作对照。结果23例实验患者RGP验配适应时间为2~6d不等。至实验结束,近视球镜度RGP组增加-0.36Ds,柱镜度下降0.12Dc;框架镜对照组球镜度加深-0.81Ds;柱镜度无显著变化。实验末两组球镜度值比较,组间差值为0.55Ds(配对t检验P〈0.05)。矫正视力:RGP组与框架镜对照组实验前后自身对照,分别上升0.52±0.22和0.12±0.05(五分对数视力)。p-VEP检测:实验始末两组自身对照,P100波振幅无显著变化,P100波潜伏时两组分别缩短13ms和8ms,差异有显著性(配对t检验P〈0.01)。结论年龄≥12周岁青少年屈光不正性弱视可选择性地使用RGP,以提高弱视眼的矫正视力。
Objective To investigate the effects of wearing a rigid gas permeable contact lens (RGP) in teenage amblyopes who were older than 12 years and to study their refractive status and pattern-visual evoked potentials (P-VEP) during a long-term follow-up. Methods Twonty-three teenage amblyopes [ 15 eyes of 10 men, 17 eyes of 13 women, average age ( 15.26 3= 2.01)years] participated in the study. All of them had been examined to measure refractive power, visual acuity and P-VEP before and after RGPs were fitted. Each of them was requested to wear the RGPs for at least 50 weeks. As a control, the same testing was also done on mother group of 20 teenagers who wore spectacles. Results 2 - 6 days were needed to adapt to the RGPs. During three years of follow-up, there was an average inerease -0.36 Ds in the spherical component of refraction in the RGP group, and -0.81D in the spectacle group (paired t-test P 〈0.05). Cylindrical power decreased - 0.12 De on average in the RGP group but did not change in the control group. Whon compared to pre-fitting, it was found that the P-VEP was shorter in P100 wave potential time ( 13 ms in the RGP group and 8 ms in the spectacle group, paired t-test P 〈 0.01), but there was only a slight increase in the amplitde of the P100 wave. Conclusion Amblyopic teenagers can wear rigid gas permeable contact lenses on a daily basis.
