穴位电针刺激对周围性面神经损伤兔面神经核中BDNF mRNA表达的影响 被引量:19

Effects of Electroacupuncture on the Expression of BNDF mRNA in the Nucleus of Facial Nerve in Rabbits with Peripheral Facial Nerve Injury

摘要 目的:观察穴位电针刺激对面神经损伤兔面神经核中脑源性神经营养因子基因(BDNF mRNA)表达的影响,揭示针刺促进损伤面神经再生修复的机理。方法:日本大耳白兔120只,随机分为假手术组(切开左侧面部皮肤后缝合)、模型组(造模后不给任何处置)、西药组(给予维生素B1、维生素B12、地塞米松)、传统针刺组(手针针刺“翳风”“颧”“地仓”“颊车”“四白”“阳白”穴)、电针刺激组(穴同传统针刺组),每组分为术后7、14、212、8 d 4个时间组。应用神经卡压法造成面神经损伤模型。采用伊红染色的方法光镜下观察面神经核中神经元细胞形态的改变,用原位杂交方法对面神经核BDNF mRNA表达阳性神经元进行染色,并用阳性细胞表达数定量分析。结果:穴位电针刺激后伊红染色可见神经细胞变性、坏死现象比模型组、西药组、传统针刺组明显改善,且电针刺激组兔面神经核中BDNF mRNA的表达明显多于其它4组(P<0.01),3个治疗组均有随治疗时间的延长而BDNF mRNA表达增高的趋势(P<0.01)。结论:穴位电针刺激对损伤的面神经修复有促进作用。 Objective: To observe the effect of electrcacupuncture (EA) on the expression of brain derived neurotrophic factor (BDNF) mRNA in nucleus of facial nerve of rabbits with facial nerve injury so as to reveal the mechanism of acupuncture In improving facial nerve injury. Methods: One hundred and twenty rabbits (Japanese big-earwhite) were randomly divided into control (sham-operation), model, medication, hand-acupuncture and EA groups, with 24 cases in each group which was further divided into 7 d, 14 d, 21 d and 28 d phases. Facial nerve injury model was established by cramping the facial nerve 0.5 cm to the stylomastoid foramen with a pair of forceps. “Yifeng”(TE 17), “Quanliao” (SI 18), “Dicang” (ST 4), “Jiache” (ST 6), etc were punctured with filiform needles and stimulated with hand manipulation in hand acupuncture group, and stimulated electrically with the stimulation parameters being 1 - 100 Hz in frequency, 2 V in strength and duration of 30 min. In medication group, musoular injection of the mixture solution of Vitamin B1 ( 10 mg), B12 (50 mg) and dexemethasone (DXM, 2 mg/d in the first 7 days, 1 mg/d later) was performed once daily. The treatment was given 1 week, 2 weeks, 3 weeks and 4 weeks respectively in different subgroups of every group. BDNF rnRNA expression of the nucleus of facial nerve was detected by using in situ hybridization (ISH) technique. Results: Compared with control group, the number of BDNF mRNA expression-positive neurons in 7 d, 14 d, 21 d and 28 d phases Of model group increased significantly (P〈0.05), while in comparison with model group, the number Of BDNF mRNA expression-positive neurons in the 4 phases of medication, hand-acupuncture and EA groups increased further and pronouncedly ( P 〈 0.01 ), and the effect of EA group was superior to those of medication and hand-acupuncture groups ( P 〈 0.01 ). Self.comparison indicated that compared with phase 7 d, the ruiner of BDNF mRNA expression-positive neurons in phase 28 d of each treatment group was markedly more ( P〈 0.01 ). No significant differences were found between medication and hand-acupuncture groups in the number of BDNF mRNA expression-positive neurons in the 4 phases ( P 〉 0.05). Results of hematoxylineosin (HE) staining showed a more significant improvement of neuronal degeneration in the nucleus of facial nerve in EA group. Conclusion. EA can promote the recovery of the injured facial nerve and upregulate BDNF mRNA expression in the nucleus of facial nerve in rabbits with facial nerve injury.
