摘要
目的旨在探讨脑卒中后抑郁障碍(PSD)患者前额叶皮质、白质及小脑的1HMRS的变化特征及规律。方法38名PSD患者按卒中发病时间分为两组,急性期组20名,慢性期组18名,与17名年龄相当的健康自愿者进行对照研究。采用GE1.5T超导磁共振扫描仪行多体素PRESS序列扫描,定位于双侧前额叶皮质、白质及双侧小脑半球。分别计算各代谢物的绝对浓度及其与Cr的比值,并比较各组间的差别。结果1HMRS显示脑卒中急(1.35±0.05)、慢性期组(1.26±0.05)左侧额叶白质区Cho/Cr较对照组(1.13±0.05)升高,且急性期组较慢性期组升高明显(P<0.05)。三组中的其余感兴趣区脑实质及小脑内各代谢物的绝对浓度及相对浓度均无显著性差异(P>0.05)。结论1HMRS能够提供PSD患者脑部各代谢物的绝对及相对浓度的代谢情况。
Objective To investigate the feature and rules of changes of post-stroke depression (PSD) in the gray and white matter of cerebral prefrontal lobe and cerebellum using ^1 H-MRS. Methods Thirty-eight patients of PSD were divided into two groups according to the time after attack, 20 in the acute group and 18 in the chronic group. Seventeen healthy volunteers of corresponding age were also examined as a controlled group. Spectra were acquired by multi-voxel PRESS sequence with GE Signa 1.5T MR/i, localizing in prefrontal lobe and cerebellum. Values of NAA, Cho, MI, Cr and ratios of Cho/ Cr, NAA/Cr and MI/Cr were calculated and compared among these three groups with one-way ANOVA. Results A signif- icant rise of ratio Cho/Cr was detected in acute (1.35±0.05) and chronic groups (1.26±0.05) by ^1 H MRS compared to the controlled group, and it displayed more significant in acute group (P〈0.05); There was no significant difference among other absolute and relative metabolites in the other ROI of cerebra and cerebellum in these three groups (P〉0.05). Conclusion ^1 H-MRS can indicate the concentration of metabolites in patients of PSD. It can provide objective information for clinical diagnosis and treatment.
