有机磷中毒致急性呼吸衰竭的临床研究

摘要 目的 探讨经鼻气管插管机械通气治疗有机磷中毒的通气方法及优势。方法 总结10年来68例中-重度有机磷中毒致呼吸衰竭患者用经鼻气管插管机械通气临床治疗。结果 68例中治愈65例,死亡2例,自动出院1例。机械通气时间1~16d,平均(9.0±1.2)d,其中36例出现中间综合征。本病的主要并发症有呼吸机相关性肺炎、鼻出血、气压伤、上消化道出血、咽痛、声嘶及迟发性神经损伤。结论 机械通气是治疗有机磷中毒所致呼吸衰竭唯一有效方法,经鼻气管插管具有患者痛苦少,易耐受,且颈部不留疤痕的优点,值得临床推广。 Objective To investigate the effect of mechanical ventilation through nasotracheal tube on patients with acute respiratory failure caused by organophosphate poisoning. Methods Mechanical ventilation through nasotracheal tube was performed in 68 patients with acute respiratory failure caused by organophosphate poisoning. Results Among 68 patients, 65 patients survived, two died and one left hospital voluntarily. Mechanical ventilation time was 1 to 16 days. Average time is (9.0 ± 1.2) days. Intermediate syndrome (IMS) occurred in36 patients. The major complications were mechanical ventilation related pneumonia, nasal bleeding, barotrauma, hemorrhage of upper digestive tract, angina, hoarseness and delayed nerve injury. Conclusions Mechanical ventilation is the only effective therapy for patients with acute respiratory failure caused by organophosphate poisoning. Mechanical ventilation through nasotracheal tube had the superiority of less pain, tolerable and no scar left on neck, thus it should be recommended.
