摘要
目的探讨代谢综合征(MS)患者大动脉顺应性及颈动脉内中膜厚度的变化。方法64例MS患者和56名年龄相匹配的正常对照者,应用脉搏波速度(PWV)测定仪对所有入选者测定颈动脉股动脉的脉搏波速度(CFPWV),用B超测定颈动脉内中膜厚度,同时检测其身高、体重、腰围、臀围、血压、血糖、血脂水平,并计算体质指数(BMI)、腰臀比(WHR)。结果MS患者CFPWV和正常对照组相比明显升高[(9.7±1.4)m/s与(7.9±1.3)m/s,P<0.01];MS患者颈动脉内中膜厚度高于正常对照组[(0.77±0.12)mm与(0.66±0.14)mm,P<0.05]。Peasion相关分析显示,MS患者颈动脉内中膜厚度(IMT)与年龄、腰围、收缩压、脉压、总胆固醇脂蛋白、甘油三酯、PWV呈显著正相关(分别为r=0.39,r=0.31,r=0.29,r=0.23,r=0.28,r=0.32,r=0.31,P<0.01),与高密度脂蛋白胆固醇呈显著负相关(r=-0.26,P<0.01);脉搏波速度与年龄、收缩压、脉压、体质指数、总胆固醇、甘油三酯、IMT呈显著正相关(分别为r=0.42,r=0.29,r=0.26,r=0.24,r=0.28,r=0.34,r=0.31,P<0.01),与体重呈正相关(r=0.19,P<0.05),与高密度脂蛋白胆固醇呈显著负相关(r=-0.23,P<0.01)。结论MS患者CFPWV及内中膜厚度均明显升高,已经存在大动脉顺应性的减退;MS患者的大动脉顺应性的变化与年龄、血压、肥胖及血脂紊乱有关,且CFPWV与内中膜厚度有明显的相关性。
Objective To investigate the changes of large arterial compliance and carotid artery intimamedia thickness (IMT) in patients with metabolic syndrome(MS). Methods There were 64 patients with metabolic syndrome and 56 age -matched control subjects. Their carotid-femoral pulse wave velocities ( C-FPWV ) were measured by the Complior SP and their carotid artery IMT were detected by B-mode ultrasound. At the same time their height,weight, waist circumstance, hip girth, blood pressure, blood glucose, blood lipid , BMI and waist/hip ratio (WHR) were measured. Results In comparison with the control group, the patients with MS had higher C-FPWV [(9.7±1.4)m/s vs (7.9±1.3)m/s,P 〈0.01]and higher carotid artery IMT [(0.77±0. 12)mm vs (0.66± 0.14 ) mm, P 〈 0.05 ]. Peasion correlation analysis demonstrated that the carotid artery IMT was positively correlated with age, WHR, systolic blood pressure ( SBP ), pulse pressure ( PP ), total cholesterol ( TC ), triglyceride (TG) and PWV(r=0.39,r=0.31,r=0.29,r=0.23,r=0.28,r=0.32,r=0.31,P〈0.01) but negatively correlated with high-density lipoprotein cholesterol ( HDL-C ) ( r = - 0.26 ; P 〈 0.01 ). Peasion correlation analysis also demonstrated that C-FPWV was positively correlated with age, SBP, PP, BMI, TC, TG , and IMT ( r = 0.42, r = 0.29, r = 0.26,r = 0.24, r = 0.28, r = 0.34, r = 0.31, P 〈 0.01 ) and was positively correlated with weight ( r = 0.19, P 〈 0.05 ), but negatively correlated with HDL-C ( r = - 0.23, P 〈 0.01 ). Conclusion There is the decrease of large arterial compliance in patients with metabolic syndrome. The changes of large arterial compliance are correlated with the age,blood pressure,obesity and the disorder of blood lipid. The C-FPWV in patients with metabolic syndrome is significantly correlated with carotid artery IMT.
