摘要
目的 探讨BiPAP鼻罩式双向正压通气治疗急性心肌梗死并发急性肺水肿的效果。方法 将60例急性心肌梗死并发急性肺水肿病人分为2组。BiPAP组30例,在常规治疗的基础上采用BiPAP ST-D-30呼吸机经鼻罩双向正压通气治疗;对照组30例采用常规治疗。观察治疗前后两组病人血气分析、血压、心率和呼吸频率的变化。结果 治疗后BiPAP组呼吸频率、心率、PaCO2均低于对照组,PaO2和SO2高于对照组(t=2.69~6.89.P〈0.01);BiPAP组治疗总有效率为100.00%.对照组在相应时间治疗总有效率仅为64.54%。结论 与常规治疗相比,BiPAP鼻罩式双向正压机械通气可迅速改善急性心肌梗死病人的心功能.并更快改善肺水肿和低氧血症。
Objective To investigate the effect of bilevel positive airway pressure (BiPAP) mechanical ventilation by nasal mask on acute myocardial infarction with acute pulmonary edema. Methods This study included 60 patients with acute myocardial infarction with acute pulmonary edema, in which, 30 patients received BiPAP mechanical ventilation treatment (BiPAP group) and another 30 patients received only general treatments (control group). Before and after the treatment, the blood gas index, blood pressure, heart rate and breathing rate were recorded. Results Breathing rate, heart rate and PaCO2 were found to be significantly lower in BiPAP group, and PaO2 and SO2 were significantly higher (t=2.69 6.89,P〈0.01). The total effective rate in BiPAP group was 100, 00%, while only 64.54% in the control group. Conclusion Comparing with conventional thera pies, BiPAP mechanical ventilation can more quickly improve cardiac function, pulmonary edema and hypoxemia of patients with acute myocardial infarction.
