摘要
目的 探讨老年冠脉造影人群先天性冠状动脉畸形(CCA)的发生率、类型及其临床意义。方法2000年1O月~2005年12月共940例年龄≥60岁的老年患者在我院行冠脉造影,对其中检出有CCA患者的临床资料进行回顾性分析。结果 940例患者共检出CCA27例(发生率2.87%),包括冠脉起源和分布异常18例(占66.7%),冠状动脉瘘9例(占33.3%)。按CCA可能导致的临床后果分类,良性畸形占70.4%(19/27),有潜在危险畸形占29.6%(8/27);对后者随访6个月~5年,未发现畸形动脉与患者心脏事件有明确关系。结论 老年人CCA并非少见,通常在冠脉造影时被发现,但大多数CCA并不导致明显临床后果。
Objective To determine the pattern and incidence of congenital coronary anomalies (CCAs) in the elderly undergoing coronary angiography, and evaluate their clinical implication.Methods To review retrospectively the chnical and angiographic features of patients with CCAs identified from 940 elderly (age≥60 years old) patients undergoing coronary angiography from October 2000 to December 2005. Results Among the 940 patients, 27 patients (2.87%) had CCAs, including 18 cases (66.7%) with anomalies of origin and distribution and 9 cases (33.3%) with coronary artery fistulas. Depending on their pathologic consequences, the 27 CCAs were classified as benign anomalies ( 19/27,70.4% ) and potentially serious anomalies (8/27,29.6%). During 6 months to 5 years of follow-up, there were no cardiovascular events related to potentially serious anomalies. Conclusion Elderly CCAs are not very uncommon and are usually casual findings of diagnostic angiographic studies. Most coronary anomalies de not result in significant clinical events.
